Job summary
We are looking for a Salaried GP to join our established clinical team for two sessions per week.
The successful candidate will provide high-quality, patient-centred care to our registered population and manage a varied general practice caseload. You will have the opportunity to work alongside an experienced and supportive clinical and administrative team and contribute to the continued development of our practice.
Main duties of the job
Provide high-quality, evidence-based care to our patients

The Continuing Healthcare Operations team provides administrative and business support to a large team of skilled clinicians who provide Continuing Healthcare services to patients across Norfolk and Suffolk.
Continuing Healthcare (CHC) is a package of care, arranged and funded by the NHS, for patients who have been assessed as having a primary health need. The CHC process involves assessing individuals with ongoing health and care needs to determine whether they are eligible for NHS-funded support, either in their own home or in a residential or nursing home setting.
The Operations team is a busy administrative function, supporting with a wide range of duties, including dealing with enquiries from patients, their families and health and social care professionals. Excellent customer service skills, the ability to organise and prioritise tasks and work effectively as a team are key skills required for this role.
Main duties of the job
Main Duties
As an Administration Assistant in the CHC Operations team you will provide comprehensive administrative support to the wider CHC team. This will include managing correspondence, telephone enquiries and email communications from patients and their families, accurately updating electronic records and databases and coordinating meetings and appointments. You will prepare documentation and outcome letters and ensure that any queries relating to cases are dealt with in a timely and efficient manner.
You will work as part of a team that processes personal and sensitive information and as such will be expected to uphold high professional standards in your communication with patients, their families and health and care professionals. Working as part of a team, you will adopt a flexible approach to switching between tasks, providing cover when required across different administrative and business tasks.

The contract is permanent

Main duties of the job

Undertake a variety of administrative duties to assist in the smooth running of the practice.

The successful candidate will join an enthusiastic team of breast specialists within the division of General Surgery, but will not be required to be part of the General Surgery on-call rota. The appointee will work with a multidisciplinary breast team delivering breast services and within the Health Board. Main duties of the job

An exciting opportunity has become available for a Band 5 Well-being Practitioner to work within our Carmarthenshire Primary Mental Health Service.
Individuals will be expected to be enthusiastic and committed to provide well-being and mental health support to help address locally identified need and sustain capacity within wider Primary Care Mental Health Services. Well-being Practitioners will deliver timely interventions in the community, which will ensure early intervention, preventing many individuals from requiring secondary mental health services.

Main duties of the job
The post holder will use a range of communication methods adaptable to the needs of individuals, families/carers and partner organisations to support individuals living with symptoms of mild mental illness.
The post holder will provide support to the Service at large, to enable the best possible care outcomes for those experiencing mental health distress and connect individuals with local Mental Health and well-being services where appropriate, including liaising with partner organisations such as GP's, Primary Care, WAST, A&E, Police and Third Sector etc.

On average, the service sorts (via its own mail sorting hubs), delivers and collects over 120,000 items of mail/medical notes per week, and transports in excess of 5 million movements of samples of Blood Products and Pathology in accordance with Accord Dangereux Routier, Good Manufacturing Practice & legislative requirements per year. As a Health Courier Service Driver/Storekeeper you will be responsible for stock items in stores centres and also for the safe and secure transport, distribution and replenishment of NHS Pathology, Blood/Blood Products, Secure and Non-Secure Stationery, Laundry, Mail/Records and Stock items such as medical consumables (e.g. needles/syringes/containers) that are distributed to primary health care sites; (Hospitals, GP practices, pharmacies, opticians, dental surgeries) across NHS Wales.
