Job summary
Due to staff retirement, we are recruiting for a Care Navigator to join our team. The role will be part time, working 25 hours per week with the following shift pattern: Monday to Friday, 13:30pm-18:30pm.
Working within the Robin Lane reception team, the post-holder will help provide a friendly and welcoming service for all patients. The Care Navigator will support patients to access the right health care in a professional and efficient manner. Follow practice processes to actively signpost patients to the most appropriate healthcare professional and service, whilst handling any complex queries and challenging situations in a professional manner, ensuring any issues are resolved where possible, at the first point of contact. Assist both clinical and admin colleagues with the smooth running of the reception area and surgery building.The post-holder is expected to comply with the employment policies and procedures, with due regard to clinical and information governance policies and procedures, and any other policies and procedures necessary for the safe and effective discharge of the duties contained within this document.

Main duties of the job
- To welcome patients presenting either in person or by telephone; ensuring you respond promptly and positively.
- Book appointments for patients and cancel appointments where needed.
- Support patients in navigating to the most appropriate service, which may include informing patients of external health care providers and online services.
- Effectively handle all queries that present at the practice; problem solving, signposting and escalation of matters, as appropriate.
- Ensure patient records are accurate and kept up to date and that all patient contacts are documented in the system using the Care Navigation Template.
- Initiating contact with and responding to requests from patients, other team members and associated healthcare providers.

Job summary
The role is to provide leadership when working on shift within the IUCS contract, including line management and development of all Senior Care Navigators. Key Tasks
- Provide a smooth running comprehensive administrative service for the Care Navigators and Clinical staff to all aspects of the Integrated Urgent Primary Care Services. The Senior Care Navigator will participate in a 4 week rolling rota which will include earlies, days, evenings, weekends and bank holidays with the occasional need to work night shifts.
- Organise and supervise the non-clinical staff to ensure that all available resources are managed effectively.
- Manage problem situations as and when they arise. These could include technical, administrative and staffing issues and queries or complaints from patients and their carers.

The Old School Surgery and UWE Healthcentre Frenchay, are looking to appoint a highly motivated Care Navigator to enhance and support our hardworking & friendly reception team.
This is a Part time role and will be made up of a mixture of early shifts (7.00am start), and late shifts (7.00pm Finish) Monday - Friday. You MUST be available to work a blend of these shift times (we'll consider all work pattern requests). This will also include very occasional Saturdays.

Job summary
We are recruiting a Primary Care Based Lead Mental Health Practitioner (MHP) working across GP practices within Central Norwich Primary Care Network to join our existing team.
This is a jointly funded role through the Additional Roles Reimbursement Scheme (ARRS), supporting NSFT and Primary Care to deliver Place-Based Care. Main duties of the job
Based within the Quality Improvement Team (QIT), you will play a pivotal role in helping the Practice deliver high quality, outcome-focused care. You will take ownership of allocated patient recall pathways and lead or support a range of clinics, including PSA monitoring, Diabetes Year of Care reviews, Severe Mental Illness (SMI) health checks, as well as contributing to joint heart failure review clinics.
This role offers a true blend of clinical and non-clinical responsibilities. Alongside data-led recall and quality improvement activity, you will deliver hands-on HCA-led care, carrying out elements of Diabetes Year of Care reviews and SMI physical health checks in line with national guidance and practice protocols.

We are recruiting a Primary Care Based Lead Mental Health Practitioner (MHP) working across GP practices within Kings Lynn PCN in West Norfolk.

Job summary
A fantastic opportunity has arisen for a dynamic, experienced and capable Chartered Physiotherapist with a flexible approach to undertake the role of Physiotherapist Specialist practitioner working in Primary Care. The vacant post is within Denbighshire cluster areas.
The post holder will be expected to work as an autonomous clinical specialist practitioner, with an extended scope of physiotherapy practice to be first contact physiotherapist working within primary care.
Support and appropriate training will be available to the successful candidate. The post holder will work closely with GP's, ANP's and other clinical staff within the GP surgeries along with the CMATS's staff within the Central IHC of Betsi Cadwaladr University health Board.
The physiotherapy service in BCUHB offers a whole career pathway to Band 8A as a clinical advanced practitioner. Service improvement and development is encouraged and all staff have access to regular supervision and training.

Main duties of the job
Provide First Contact Physiotherapy Assessments and treatment of MSK patients.
Onward referral of patients to appropriate services and for appropriate investigations.
Develop opportunities for practice which extends the scope of traditional physiotherapy
Training and support of physiotherapists and support staff to deliver high quality, evidence based service provision

Job Summary:

Health & safety
The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control Policy and published procedures. This will include:
