Responsibility for People Management

Provide clinical supervision and line management for specified members of staff within the MDT and ensure all other line management is completed within the team.

To manage the overall team on a day-day basis ensuring that the skill-mix is appropriate to the needs of the patients.

Our GPs hold patient lists, supporting continuity of care and long-term relationships with patients and families. The successful applicant will join a team of 10 GPs, supported by highly experienced Advanced Nurse Practitioners, Clinical Pharmacists, nurses, Care Co-ordinators and Social Prescribers.

Planned Care sessions typically include 25 core patient contacts across the day, with additional GP-controlled continuity/follow-up slots. Protected time is built in for administration and review, alongside daily coffee breaks and flexibility in managing workload.

Practice visits are managed by the duty On the Day Team, primarily through our Visiting ANP, protecting routine Planned Care sessions. Planned visits remain welcomed where they support continuity of care.

PHL Group are commissioned by Staffordshire & Stoke-on-Trent Integrated Care Board to provide out-of-hours (OOH) clinical services across the county of Staffordshire. Our service offering includes telephone triage, home visiting and patients visiting one of our seven primary care centres. These services are delivered between 18:30 08:00 Monday to Friday and from 08:00 08:00 across weekends and bank holidays. Service operating times are variable based on patient demand and access to premises.

The post holder will support the three commissioned services, ensuring they are safe, efficient and patient-focused. They will work closely with the Medical Lead, Clinical Lead and Business Delivery Manager and ensure patients receive optimal care in the correct setting within the healthcare system.

The service is staffed by a team of GPs, Advanced Nurse/Clinical Practitioners, Nurses, Paramedics and a non-clinical workforce.

Clinical Duties

The GP will work with a multi-disciplinary team to ensure appropriate care is delivered in the right place, at the right time. The post holder will be expected to:

Maintain the highest clinical standards for all patients accessing the service.

Elucidate the primary reason for the patients referral or attendance within the OOH service.

Assess and treat patients presenting with clinical needs which can be met by primary care.

Provide emergency care while waiting for emergency transfer of patient when appropriate

Provide advice and support to colleagues and the multi-disciplinary team.

Manage the flow of patients by balancing the clinical need of the patient with the capacity of the provider to deliver the necessary response.

Support both injury and illness pathways.

Maintain contemporaneous medical records for all patients consulted with.
