To be responsible for undertaking a wide range of administrative duties and the provision of general support to the surgery team. Main duties of the job

- Monitoring the practice appointment system
- Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
- Providing a front of house first point of contact for patients within the reception area
- Processing repeat prescription requests
- Processing Accurx Online Triage bookings
- Enter and read SNOMED CT code data taken from clinical letters
- Patient data entry allocation & collation; processing and recording information in accordance with CGPN procedures
- Offer general clerical assistance to Cherry Hinton Medical Practice in all aspects of practice delivery and project a positive and friendly image to patients and colleagues, either in person or via the telephone
- Supporting Secretarial cover as required
- Support other members of staff, providing cover during staff absences
- Action incoming emails when necessary
- Always observe patient confidentiality

As a Senior Practice Nurse, you will help oversee day-to-day nursing operations and deliver safe, effective, patient-centred care whilst helping improve patient outcomes.
We are looking for a confident clinician with strong nursing skills who can work effectively both independently and as part of a team. The role is also supported by an experienced wider federation Senior Management Team, including a Lead Nurse. We welcome any queries relating to the role and informal visits to the practice.
Main duties of the job
The successful candidate will work closely with the practice team to provide a wide range of nursing services, including long-term condition management, health promotion, infection prevention and control, immunisations, cervical screening, wound care and clinical governance. They will provide help with day-to-day support to the nursing team, promote safe and effective patient care, contribute to quality improvement, and help ensure that services are delivered in line with practice policies, professional standards and local and national guidance.

Practice Nurse with focus on Chronic Disease Management Cherry Hinton and Brookfields Medical Centres
We are looking for a skilled and enthusiastic Practice Nurse with experience in Chronic Disease Management to join our supportive team at Cherry Hinton and Brookfields Medical Centre. The ideal candidate will have experience in managing patients with either respiratory diseases (Asthma and COPD) and/or diabetes.
We are based in South Cambridge and provide care to approximately 10,000 patients. The practices have great transport links for those commuting from in and around Cambridge. This an excellent opportunity for a dedicated nurse who is passionate about high-quality patient care and keen to help support and develop our services.
This role will focus on the provision of chronic disease care. We welcome applications from clinicians with strong nursing skills who have experience in reviewing and providing appropriate management, advice and health promotion in line with local and national policies. The role will be supported by a comprehensive team including a lead nurse and clinical lead within the practice. We welcome any queries relating to the role and informal visits to the practice.
Main duties of the job
The successful candidate will work closely with the practice team to provide evidence-based, high-quality care to patients diagnosed with chronic diseases. The role will also assist the nursing team with routine care including immunisations, cervical screening and wound care.
