Job summary
The Clinical Practitioner will work across Severnside Integrated Urgent Care (IUC), including The Burren, Out of Hours, Clinical Assessment Service (CAS), Clinical Medication Delivery Unit (CMDU) and Frailty Assessment and Coordination for Emergency and Urgent Care (ACE).
The Burren is an innovative collaborative service bringing together secondary, community, urgent and primary care within an intermediate care setting. The post holder will be an autonomous prescribing clinician, providing high-quality primary medical care while working within their professional scope of practice.The role involves managing the day-to-day clinical needs of patients discharged from secondary care into community placements with rehabilitation needs. The successful candidate will have advanced clinical assessment and decision-making skills, be confident in managing clinical risk, and work independently while contributing to excellent patient outcomes. Experience in urgent care and chronic disease management would be advantageous.
Supported by the Nursing and AHP Leadership Team and Primary Care GPs, the Clinical Practitioner will provide patient-centred care and ensure individual health needs are met.
As a key member of the multidisciplinary team (MDT), the post holder will participate in ward rounds and work closely with Nurses, GPs, Therapists, Social Care and Operational colleagues to support patients and deliver the right care at the right time.
Main duties of the job

Job summary
The Lead Clinical Practitioner (LCP) is a senior leadership role within Severnside Integrated Urgent Care (IUC), providing advanced clinical practice across The Burren, Out of Hours, Clinical Assessment Service (CAS), Clinical Medication Delivery Unit (CMDU) and ACE services. The role combines clinical duties with leadership responsibilities, including supervision, mentoring, clinical governance, audit, service improvement and patient safety.
Within The Burren, the post holder will work as an autonomous prescribing clinician, delivering primary medical care to patients discharged from secondary care who require rehabilitation and ongoing clinical support. The role requires advanced assessment, clinical decision-making, risk management and chronic disease management skills, supported by the Nursing and AHP leadership team and primary care GPs.
The post holder will act as a key member of the multidisciplinary team, participating in ward rounds and managing routine and urgent patient needs. Working closely with nurses, AHPs, GPs, therapists, social care and operational teams, the LCP will support integrated care, improve patient outcomes and ensure patients receive the right care at the right time. The post includes 16.75 clinical hours and 6 leadership hours per week.

Job summary
DRIVER
To primarily transport clinicians to the homes of patients in order to provide medical assistance. The post holder will also be responsible for the maintenance of the vehicle during each shift. At all times drivers must carry out duties in a safe and legal manner ensuring that all vehicles are maintained and driven within current driving regulations and codes, principally the Road Traffic Act and Highway Code.
Please note: this is a hybrid role working as a driver and a host
HOSTThe purpose of this role is to provide a front of house reception service at Brisdoc's treatment centres and promote high standards of patient experience. The main responsibilities include: welcoming patients on arrival and supporting them through their patient journey in the service; answering internal phone calls within the agreed BrisDoc standards and recording all necessary information accurately for other team members to use; performing accurate record keeping and all other front line reception duties as required.
Main duties of the job
- Ensuring the car has adequate fuel for the shift. When necessary, re-fuelling the car using the company fuel card.
- Responsibility for safely loading and unloading the appropriate medical equipment and supplies in the vehicle that may be required by the mobile clinician and driver, whilst adhering to all manual handling guidelines.
- Completing the drivers check list at the start of each shift ensuring all necessary equipment/supplies are available and functional in the car.
- Maintaining the cleanliness and general upkeep of the vehicles and report any faults immediately to the Shift Manager and completing the fault notification sheet.
To drive the clinician to patients' homes / place of residence safely and within the current road and traffic act regulations noting that this is not an emergency service and speed limits and laws in accordance with the Highway Code must be followed at all times. Responsibilities include:
- Accurately recording all call details of visits, returning all Driver Log sheets to base at end of shift.
- Driving deliveries to other destinations at the request of the Shift Manager. This may include delivering supplies, prescriptions, medication, etc between sites.
