Main duties of the job
Bolton GP Federation is looking to recruit a Clinical Pharmacist on behalf of our Primary Care Networks (PCNs).
You will have a broad portfolio of duties that originate from the requirements of the PCN contracts, including the Directed Enhanced Services (DES) and Impact & Investment Fund (IIF).
PCN Pharmacists provide primary support to general practice staff with regards to prescription and medication queries.

Key Responsibilities and Duties:
The Clinical Pharmacist will undertake work in line with PCN and directed priorities. The following are the core responsibilities of the role:
- help support the repeat prescriptions system
- deal with acute prescription requests
- complete medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).
- provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
- will also take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

Job Role - may include the following:
Management of medicines at discharge from hospital
- To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
- Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g., those with medicine compliance aids or those in care homes).
Risk stratification
- Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
Unplanned hospital admissions
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.

Main duties of the job
Care coordinators provide extra time, capacity and expertise to support patients. You will work closely with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload of patients, making sure that holistic support is made available to them and that their complex needs are addressed. You may also be asked to contribute towards other administrative tasks within the practice or PCN.

Main duties of the job
We have an exciting opportunity for a Nursing Associate to join our Primary Care Network team. The Nursing Associate will work under the supervision of the Practice Nursing Team within the allocated PCN. Leadership and support will be provided by the PCN Manager, Clinical Director, and member practices. The Nursing Associate will work within the community to support the everyday running of the PCN practices. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience. The Nursing Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications.

- Work within the PCN to provide and monitor care, alongside the wider PCN team, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision
