Role: Paramedic General Practice

Reports to: GP Partner

Hours: 24 hours per week Contract: Fixed term maternity cover Salary: Depending on experience

Job Summary

The Paramedic will work as part of the multidisciplinary team (MDT) within Balmoral Surgery to provide high quality, patient centred care, primarily for onsite urgent/same day presentations, with a small number of home visits as agreed.

The post holder will:

See and manage appropriate same day / urgent cases on site, within an agreed scope of practice and under GP supervision.

Undertake a limited number of home visits for suitable patients, as agreed in the rota.

Support GPs by safely managing appropriate cases, while escalating undifferentiated, complex or high risk presentations to a GP.

Overall clinical responsibility for patients remains with the GP partners.

Key Responsibilities

Clinical

Within an agreed and regularly reviewed scope of practice, and with access to a named GP supervisor, the Paramedic will:

Provide onsite same day / urgent care for appropriate presentations, following practice protocols and escalation pathways.

Undertake face to face, telephone and video consultations for:

Minor illness and injuries

Selected long term condition reviews, where agreed and within competence.

Perform comprehensive clinical assessments , including history, examination and observations.

Formulate working diagnoses and management plans for conditions within competence, using evidence based guidelines and practice protocols.

Provide home visits for suitable patients (e.g. acute but stable presentations, followup of known patients) as agreed in the practice rota, ensuring:

Clear pre visit briefing where needed

Timely documentation on return to the practice

Escalation to a GP if concerns arise.

Implement management plans including:

Self care advice and safety netting

Use of practice protocols and patient information

Arranging appropriate followup or GP review.

Recognise red flags and clinical risk , and promptly escalate to a GP when:

Presentations are undifferentiated or complex

There is diagnostic uncertainty

The patient is clinically unstable or deteriorating.

Request and interpret basic investigations within agreed scope (e.g. simple blood tests, urinalysis, ECG recording, basic observations), and escalate abnormal results to a GP.

Make referrals to community and social care services in agreement with a supervising GP and within local pathways.

If the post holder is an independent prescriber (or becomes one), prescribing activity must be explicitly agreed, supervised and audited by the GP partners, and remain within their competence and practice governance requirements.

Non Clinical / Administrative

Document all patient contacts and clinical decisions contemporaneously in the clinical record.

Manage clinical administration related to their caseload, such as:

Coding and summarising relevant information

Actioning agreed test results within scope

Drafting referrals for GP review and sign off where required.

Participate in clinical audit, quality improvement and service development projects relevant to urgent care and home visiting.

Contribute to practice meetings , including MDT meetings, clinical governance and significant event reviews, as appropriate.

Support health promotion and prevention initiatives (e.g. lifestyle advice, vaccination campaigns) within competence.

Professional and Governance

Work at all times within HCPC standards of conduct, performance and ethics, and within the scope of practice defined by the practice.

Maintain professional registration with the HCPC and comply with all revalidation and CPD requirements.

Participate in regular clinical supervision with a named GP supervisor.

Engage in an annual appraisal and agree a personal development plan aligned with practice priorities.

Adhere to all practice policies , including:

Safeguarding (adults and children)

Infection prevention and control

Information governance and confidentiality

Health and safety and incident reporting.

Supervision and Support

The Paramedic will have a named GP clinical supervisor responsible for day to day clinical support and oversight.

The post holder will have:

Regular scheduled supervision sessions

On the day access to a GP for advice and escalation

Opportunities to discuss complex cases and learning needs.

The GP partners retain ultimate responsibility for clinical governance and for ensuring that the Paramedics scope of practice is safe, clearly defined and regularly reviewed.

Essential Qualifications

Degree / Diploma in Paramedicine or equivalent.

HCPC registration as a Paramedic.

Completion of the two year Consolidation of Learning period as a newly qualified paramedic.

At least three years experience as a Band 6 (or equivalent) paramedic.

Evidence of ongoing CPD relevant to urgent and primary care.

Desirable Qualifications

Masters level modules or equivalent in advanced clinical practice / paramedic practice.

Non medical prescribing qualification (V300), if prescribing is to be part of the role and supported by the practice.

Additional training in minor illness, frailty, long term conditions or palliative care.

Essential Experience

Substantial post registration experience in urgent / emergency care (e.g. ambulance service, urgent care centres, ED).

Experience of autonomous clinical assessment within defined protocols and escalation frameworks.

Experience of working within a multidisciplinary team .

Experience of managing acutely unwell adults and recognising deterioration.

Desirable Experience

Previous experience in general practice or community based services.

Experience of home visiting or care of housebound/frail patients.

Involvement in audit, quality improvement or service development .

Knowledge and Skills

Essential:

Strong clinical assessment and decision making skills within scope of practice.

Ability to recognise red flags and clinical risk and escalate appropriately.

Understanding of how general practice operates, including appointment systems and triage.

Excellent communication and interpersonal skills .

Competent IT skills, including use of GP clinical systems (training can be provided).

Ability to work autonomously within defined parameters , and as part of a wider MDT.

Desirable:

Knowledge of NICE guidance and local clinical pathways relevant to urgent and primary care.

Main Purpose of the Role

To provide high-quality, patient-centred general medical services to the practice population, working as part of a supportive and collaborative multidisciplinary team.

The postholder will provide clinical assessment, diagnosis, treatment and ongoing management of patients presenting with a wide range of medical conditions, while contributing to the continued development of the practice and wider Primary Care Network.

Clinical Responsibilities

• Undertake approximately 25 patient consultations per day, with appointments scheduled at 15-minute intervals.
• Provide appropriate clinical assessment, diagnosis, treatment and management of patients presenting with both acute and chronic conditions.
• Manage patients holistically, considering their physical, psychological and social needs.
• Refer patients appropriately to secondary care and other healthcare professionals where clinically indicated.
• Prescribe safely and appropriately in accordance with current guidance and practice policies.
• Review and manage results, clinical correspondence and other investigations within agreed practice systems.
• Provide appropriate follow-up and safety-netting advice to patients.
• Participate in chronic disease management and preventative healthcare where appropriate.
• Maintain accurate and contemporaneous clinical records.
• Work collaboratively with the wider multidisciplinary team, including the practice nursing team, ACP, Paramedic and PCN-supported services.

The GP will be expected to:
