Job summary
We are looking for a GP to work on alternate Saturdays for routine and long term condition reviews, providing enhanced access hours for our Primary Care Network (PCN), predominantly from our Asplands site.
At Ascent PCN, we offer something a little different from the usual weekend GP role. You'll be joining a friendly, welcoming team where our weekend clinics are staffed by a regular group of clinicians and support staff. This familiarity creates a strong sense of camaraderie, making weekends feel like a genuine team rather than simply an extra session. Appointments are longer and the surgery is calmer than during the week.
Ascent PCN brings together three practices and around 30,000 patients across four sites, spanning rural and urban communities and a wonderfully diverse range of socioeconomic backgrounds. This gives you the opportunity to experience the full breadth and variety of general practice within a single role.
Whether you enjoy the complexity of managing multimorbidity, the variety of undifferentiated presentations, or simply the opportunity to work with a diverse patient population, weekends will feel quite different to the norm.
You'll be supported by an experienced and approachable team. We value collaboration, continuity and a positive working atmosphere and we'd love to welcome a GP who shares those values.

Main duties of the job
Weekend clinics are comprised of usual GP duties including face to face and remote consulting, conducting long term conditions reviews, planned routine reviews and proactive care. Appointments on a weekend are 15 minutes long and there is no current requirement for home visits to be done. The sessions are "by appointment only".
The GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced Services, including:
- In person consultations
- Telephone consultations
- Checking and signing prescriptions
- Dealing with queries, paperwork and correspondence as required by weekend services
- Clinical administration including processing of results and coordinating referrals for specialist review
- Leading, supporting and advising the allied health professionals working in clinics during the clinic session.
- You may be the only GP on site so need the confidence and experience to cope with that.

We are a 3 practice Primary Care Network covering around 30000 patients across 4 sites, varying from rural to urban, affluent to deprived populations.

We are looking for an enthusiastic, organised and proactive individual to join our practice team as Digital and Operations Support.
This is a varied role combining digital transformation, operational support and data management. You will help the practice make the best use of technology by supporting staff, monitoring performance against contractual requirements and identifying opportunities to improve efficiency and patient care.

Main duties of the job

Operations support will also be part of the role, monitoring the achievement of the practice against key targets. It will also support the team with extraction of data from the pateient record system.

Job summary
The role exists to support the delivery of the Primary Care Network agenda, ensuring effective functioning to meet contract requirements. It plays a pivotal role in the delivery of high-quality primary healthcare across a network of 3 General Practices. As a manager, this role will form an integral part of the PCN leadership team working alongside the Clinical Director, Business Manager and PCN board members. It is vital that the transformation within the PCN aligns the PCN within the developing Neighbourhood.
The successful candidate will have operational responsibility for various functions of the PCN. They will be responsible for the delivery and monitoring of services, digital platforms and transformational projects committed to or commissioned by the PCN. They will also provide oversight of the financial management, resourcing and regulatory compliance of the work of the PCN, ensuring that transformation falls within financial constraints. Their knowledge of systems and digital solutions will guide the PCN to optimise efficiency and support clinical functions. The ability to analyse data will be required to direct clinical and non-clinical reviews, processes and strategic decisions.
Coordinating the successful achievement of governance requirements and working with Network Members to translate national and local policy into meaningful changes and will support practices to achieve improved outcomes for the local population.

Main duties of the job

About us
We are looking for a proactive transformation and operational manager to join our friendly PCN management team. The ideal candidate will have the ability to work and prioritise tasks independently and contribute to the wider PCN agenda and strategy, including Neighbourhood planning. You will already have the skills, knowledge and work experience to meet the requirements of this role and be able to communicate effectively with colleagues and external stakeholders.
This is a permanent role for the Ascent Primary Care Network, a group of three GP surgeries working together for their communities across Milton Keynes and a small part of Bedfordshire. The PCN consists of Asplands Medical Centre (Woburn Sands and Woburn), Walnut Tree Health Centre and Fishermead Medical Centre. Although the role of Manager will primarily operate from the Asplands Medical Centre offices in Woburn Sands, there is requirement to travel between the Network sites as well as home working on occasion. The role will offer comprehensive leadership workflow and administration to support to the PCN Clinical Director and Board, as well as supporting the functions of the wider clinical team (ARRS team) across the PCN. - To be responsible for the oversight of the Networks financial systems/payments/forecasting and keep the PCN within its budgets

The role of the HCA is an essential part of our GP Practice team who work as a multidisciplinary team to provide a full range of Primary Medical Services.
You will be working alongside our friendly and patient focused nursing team in the delivery of nursing services, delivering care within your scope of practice to the patient population. The healthcare assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education.
Main duties of the job
The main responsibilities for our HCA team is to support the practice team in providing:
- Wound care - some complex so experience is a must with leg ulcers and complex wounds
- Phlebotomy
- Long Term Condition management - mainly diabetes checks so evidence of training in foot checks is a must
- Health promotion and NHS Healthchecks
- Vaccinations - ideally has attended the 2 day immunisation training and is competent
