As a First Contact Physiotherapist, the post holder will work as part of a multidisciplinary team across the practices of a Primary Care Network (PCN), with the PCNs GPs and other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. Job Summary

Specialty Doctor

For entry to the Specialty Doctor grade, applicants must have at least four years of post-registration training, of which at least two years must be in a speciality training programme in Emergency Medicine (forexample, ACCS) or must be able to demonstrate comparable skills and competencies.The job title of the post is Specialty Doctor in Emergency Medicine. The post is paid on the Specialty Doctor scale. Applicants wishing to work part-time would be considered. Job Plans are worked around a twenty four person rota. The job is 10 PA (typically 1.5 SPA 8.5 DCC)The post holder will be encouraged to advance their career in Emergency Medicine and will be supported inobtaining postgraduate qualifications. The department also has experience of supporting doctors in CESR(previously Article 14) applications, and again this would be encouraged for suitable experienced doctors.

Duties of the Post

The postholder is expected to work on the middle grade tier rota in the department and provide the relevantexpertise and experience necessary to ensure the efficient primary management of emergency cases. Thepostholder will be required to work with the other team members in the management of the departmentworkload. The postholder will also be expected to provide supervision to the junior members of the team.The postholder will be expected to undertake full clinical duties including the immediate care of critically illpatients attending the Emergency Department and also patients presenting with injuries both major andminor. The care will include initial resuscitation where appropriate, diagnosis, management and thetreatment, referral and discharge under the guidance of the Consultants. The referral could be to GPs to anappropriate specialist team for continued hospital care or to other services.

The postholder will be expected to follow the various guidelines and protocols issued in the department. Inthe case of major trauma cases, cardiac arrests and critically ill patients attending the department, thepostholder will be expected to act in a team leadership role, following appropriate life support guidelines.

Postholders would be expected to gain and maintain certification in ALS, ATLS etc and APLS or recognisedequivalents to these courses.In light of the changing work practices, the work programme may need to be varied in the future. This willbe done only after consultation with the incumbent of the post. As duty hours require the incumbent beingpresent in the department, no on-call or residential accommodation is offered with this post.In exceptional circumstances, additional duties outside normal rostered hours may be required to ensurethe continuation of essential patient care. So far as is consistent with the proper discharge of the aboveduties, the postholder may be required to deputise from time to time for absent colleagues.An educational mentor will be provided process to make sure that the professional and personaldevelopment of the postholder are maintained. Teaching, Audit & Research

The postholder will be expected to participate in teaching and training of junior doctors, medical students,nurses and paramedical staff as directed by the Clinical Lead. Emergencies and Unforeseen Circumstances

Study and Training

Postgraduate study is encouraged. In-service training is available and clinical meetings are held in theDepartment. The Department

The Emergency Department is located at the Royal Blackburn Teaching Hospital a newly built PFI hospitalwith fully integrated Paediatric and adult areas with Resus, PACS system, and fully supported by CCU andITU Assessments Units, X-ray, CT and MRI facilities. There is a hospital trauma team and cardiac arrest ismanaged in house.In additional to the main Emergency Department which takes primarily ambulance cases, there are 2Urgent Treatment Centres catering to minor injuries and illnesses assessing both walk-in and appointedpatients. One is situated in Burnley General Hospital and other co-located within the Emergency Department at Royal Blackburn Teaching Hospital.The Emergency Department has an 8 bedded resuscitation area (with 1 bed designated for paediatricpatients) with 42 major cubicles. The two Urgent Treatment Centres have examination cubicles, staff base,separate adult and children waiting areas, reception and all facilities to see minor injury and illness. Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.

Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.

Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.

The Targeted demand management (TDM) service supports people across Blackpool to address clinical and non-clinical factors increasing their risk of an unplanned hospitalisation.

Key aims of TDM are to provide clinical and non-clinical interventions that meet the needs of individuals identified in Blackpool with the following respiratory or cardiovascular long-term conditions. The TDM service, through reducing individual risk, will play a role in relieving pressure on acute services, through preventing health crises.
