Job summary
We are looking for a Care Navigator to join the team at Debenham Group Practice. This is a patient-facing role at the heart of how the practice runs day to day, and we are looking for someone who will bring a calm, friendly and organised approach to supporting our patients and colleagues.
Debenham Group Practice is part of Suffolk Primary Care, which gives us access to a wider range of shared expertise and organisational support, while allowing us to remain a well-regarded local practice with a strong community identity.
About us
We care for 10,000 patients across our main site in Debenham, with branch surgeries at Otley and Grundisburgh.
- A stable team, with a good mix of experience
- A strong team ethos, with people supporting each other day to day
- Use of SystmOne and askmyGP to support patient access and workflow
- Access to a broader multi-professional team through Suffolk Primary Care
We are looking for someone who:
- Is calm, warm and professional when dealing with patients and colleagues
- Listens carefully and records information accurately
- Is organised, reliable and takes their responsibilities seriously
- Works well as part of a team and is willing to support colleagues
- Has good IT skills and picks up new systems with confidence
- Has previous experience in a customer-facing or administrative role
Experience in a healthcare or NHS setting is helpful but not essential we are happy to support the right person to develop in the role.
Main duties of the job
Care Navigators are often the first voice or face a patient encounters. Whether patients contact us by phone, walk through the door, or submit a request online, you will help make sure they get the right information and reach the right place.
We use askmyGP as our primary workflow tool for clinical requests. Rather than triaging patients themselves, Care Navigators help ensure that requests are accurately recorded so that the duty GP can review and act on them promptly. You will also take turns supporting the duty GP as part of the wider Care Navigation team, and you will help signpost patients to other local services where that is the most appropriate response.
The role also covers the full range of day-to-day practice administration from reception duties and managing records, to supporting clinical and dispensary colleagues and keeping the practice running smoothly.
You will work across our sites as needed, as part of a well-established, friendly team.
We are looking to recruit for Wednesday, Thursday and Friday. Shifts fall within a working window of approximately 07:30 to 18:30 across those days. Depending on recruitment, hours may be split across more than one person.
We welcome applications from individuals who can cover all three days as well as those who are available for part of the pattern, so please do apply if mid-to-end of week working suits your availability.
About us
What we offer
- A supportive, well-established team in a friendly rural practice
- Opportunities for training and development
- Access to the wider resources and support of Suffolk Primary Care
- Access to NHS Pension
This is a role where you can make a genuine difference to patients and to the team around you. If this sounds like the sort of place youd like to work, wed be delighted to hear from you.

Job summary
This is an exciting opportunity to join the Western Dales Primary Care Network as a Social Prescribing Link Worker, working directly with patients and helping to make a meaningful difference to their health, wellbeing and everyday lives.
Working across our communities in Sedbergh, Bentham and Kirkby Lonsdale, you will support people to explore what matters most to them, identify the challenges affecting their health and wellbeing, and connect them with the right support, opportunities and services.
Social prescribing is about seeing the whole person, not simply their health condition. You will have the time to listen, build trusted relationships and work alongside patients to identify their goals, build confidence and independence, and help them make positive and sustainable changes in their lives.
You will become part of our established personalised care team, working alongside other Social Prescribing Link Workers, Care Coordinators and Health and Wellbeing Coaches, as well as colleagues across our GP practices and wider community.
This is a varied and rewarding role for someone who enjoys working with people, building relationships and finding practical and creative ways to help. No two days will be the same, and the work you do has the potential to make a genuine difference to people living across the Western Dales.
Main duties of the job
The Social Prescribing Link Worker will work proactively with patients, GP practices, multidisciplinary teams and community organisations to provide personalised, practical and compassionate support. Key responsibilities will include:
Take referrals from clinicians and multi-disciplinary teams, and work closely with other PCN team colleagues to triage those patients and allocate to the most appropriate person
Discuss the persons needs with them, based on guidance from the referrer, and identify a range of options that could assist the person to improve their independence and health and wellbeing
Strengthen community and personal resilience, focusing on what matters to me and taking a holistic approach with each individual case
Co-produce a simple personalised care and support plan to improve health and wellbeing introducing or reconnecting people to community groups and statutory services
Manage and prioritise own caseload, in accordance with the needs
Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when the persons needs are beyond the scope of the link worker role e.g. when there is a need requiring a qualified practitioner
Identify new, and work in partnership with voluntary and statutory organisations.
About us
Western Dales Primary Care Network is a collaboration of three practices; Bentham Medical Practice, Lunesdale Surgery and Sedbergh Medical Practice with a combined population upwards of 20,000.
We are looking for a Social Prescribing Link Worker to work within a team of social prescribers, care coordinators and health and well-being coaches, developing relationships and personalised care plans to support patients in reaching their identified goals.
