Job summary
We are pleased to be advertising for a Band 4 Associate Practitioner to work with us in our Enhanced Recovery and Support at Home Team. You will be working as part of a team consisting of Physiotherapy, Occupational Therapy, Nurses and Rehab Support workers assistants.
The service operates 8am-6pm 7 days a week and you will be expected to work on a rota basis to cover these hours. The team has a clinical supervision structure and there is access to comprehensive in-service training.
With a particular focus on rehab, falls and provision of equipment for patients at home, you will have experience in working autonomously as delegated by the OTs and Physios, be able to make your own assessments of patients' risks and needs, and undertake rehab treatment programmes.
You will receive direct line management from a registered therapist who will support you to develop within the role, both in terms of confidence as well as competence, and access further training and development opportunities to help you expand your skills and knowledge as appropriate.
Career progression and personal development will be supported.
As this is a community post, you must have access to a vehicle for work purposes with business insurance and, although generally rostered to work Monday to Friday, some weekend and evening shifts may be required.
Main duties of the job
- Deliver high quality care support and assessments in people's homes, under guidance of a registered therapist
- Undertake initial assessments of new patients as delegated by registered therapists, including early identification of risks and development of appropriate care plans , while promoting patient independence;
- Provide support to the community therapies teams and community rehab wards as needed
- Be responsible for own caseload, including assessment, rehab progression and discharge planning as delegated by registered therapist
- Demonstrate capacity to identify, review and risk assess basic equipment needs and issue them in the community autonomously
- Be able to travel in between patients addresses in a timely manner and organise visits efficiently
- To work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

An opportunity has arisen for an Advanced Clinical Practitioner to join our dedicated teams based on Herm & St Brelades Wards, Alderney Hospital.
The inpatients services are for Older Persons with a organic or functional mental health diagnosis.

An opportunity has arisen for the post of Associate Practitioner (Band 4) withinHistopathology at Liverpool Clinical Laboratories. This is a permanent role and is an opportunity for an existing member of staff in either LCL or LUHFT to obtain a set of highly technical laboratory skills, whilst contributing to cancer and other patient care.
This post is essential for the day to day running of the service and the successful candidate will receive the training necessary to work with other staff within the team to deliver the highest quality standard of patient care.
The directorate receives greater than 65,000 cases per year and is the largest provider of specialist Cellular Pathology services within Cheshire and Merseyside. The department consists of over 100 members of staff, including Medical Laboratory Assistants, Associate Practitioners, Biomedical Scientists, Consultant Pathologists, pathology trainees and administrative and managerial staff working together to deliver the specialist Cellular Pathology service at LCL for Cheshire and Merseyside
The department's operating hours are 07:00 to 19:00 Monday to Friday and 08:00 to 16:00 Weekends and Bank Holidays.

The successful candidate will undertake all routine tasks within the Histology laboratory according to departmental Standard Operating Procedures and policies. These tasks include:

An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner to join our Urgent Community Response, Virtual Ward and Reablement Team within Nottingham CityCare Partnership.
The Advanced Clinical Practitioner plays a pivotal role in delivering rapid, high-quality clinical assessment and intervention for adults experiencing an acute health or functional crisis in the community. The ACP provides expert autonomous practice to prevent avoidable hospital admissions, support safe and timely discharge, and enable effective reablement, ensuring people can remain safely at home wherever possible.
The postholder leads clinical decision-making within a multidisciplinary team, provides advanced assessment and treatment planning, prescribes medication where appropriate, and contributes to service development, staff training, and integrated working across health and social care partners.
The ACP carries clinical responsibility for a complex caseload, applying advanced investigative and analytical skills to lead safe, evidence based practice, support timely discharge planning across community settings, and provide specialist advice to colleagues across the multidisciplinary team.
The post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients presenting to their area by providing clinical advice and support to nursing staff and other health care professionals.

Main duties of the job
The ACP is accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. The post holder will practice autonomously within the Advanced Practitioner level of competence working within the Integrated Reablement Team (IRT)/Virtual Ward (VW)
They will assess, diagnose, treat or refer patients aged 18+, presenting with undifferentiated and undiagnosed problems.

Main duties of the job
Wessex Clinical Genetics Service is delighted to be recruiting a Band 5 Genomic Practitioner.
Based in the Clinical Genetics department within the Princess Anne Hospital, the Genomic Practitioner will work within the Genomes team to facilitate and organise whole genome sequencing testing for a wide variety patients across the Wessex region. The main function of this role is to speak to patients and their families to discuss genetic testing as well as options to be involved in research and documenting their choices. This will mostly be over the telephone, however, you may be required to see patients face-to-face in clinic or ward settings.
The Genomes team is mostly a small team of Genomic practitioners, lead by a specialist nurse. We are based within the Wessex Clinical Genetics Service and work closely with consultant geneticists and genetic counsellors, as well as many clinicians within other specialities. Our team receives over a thousand referrals a year for whole genome sequencing from many different clinicians around the Wessex region.

The successful candidate will provide high-quality, patient-centred primary care services to the practice population, undertaking a range of clinical duties in accordance with GMC standards and current best practice.
Key responsibilities will include conducting face-to-face, telephone and online consultations; assessing, diagnosing and managing acute and chronic medical conditions; prescribing safely and appropriately; reviewing and actioning test results, clinical correspondence and medication requests; and making referrals to secondary care and community services where appropriate.
The postholder will work collaboratively within our multidisciplinary team, contributing to the delivery of safe, effective and efficient patient care. They will participate in practice meetings, clinical governance activities, quality improvement initiatives and service development as required.
The role also includes maintaining accurate and contemporaneous clinical records using the practice clinical system, supporting continuity of care, and adhering to all relevant clinical, professional and organisational policies.

Our practice provides excellent care and excels in treating and caring for patients with long term conditions. We provide minor surgery clinics, joint injections, women's health clinics, menopause clinics, baby clinics, LTC clinics and much more! We are consistently high QoF achievers year on year which is all down to our great team and their work ethic. We had a recent CQC inspection and was rated Good' over all with a Good in all areas.
