Job summary

We are looking for compassionate and experienced Band 6 Registered Nurses to join our new Worcestershire 24/7 Palliative and End of Life Care Advice Line service. This is an exciting opportunity to be part of a countywide service providing timely advice, telephone triage, symptom management guidance, reassurance and care coordination for patients deemed to be in their last year of life, their families, carers, care homes and professionals.
Working as part of an integrated model with our Single Point of Access (SPA), Evening and Night Nursing, Neighbourhood Team and Specialist Palliative Care services with strong collaborative working with Primary Care and other system partners, you will help people remain in their preferred place of care and reduce avoidable crisis escalation and avoidable Emergency Department attendances or hospital admissions.
The role is clinically focused and will be co-located with other clinicians with the ability to escalate to more senior clinicians where appropriate. There will be only one clinician covering this phoneline so the ability to deliver the service confidently is essential.

In this role, you will provide telephone triage, clinical assessment and advice to patients deemed to be in their last year of life, as well as their families, carers, care homes and healthcare professionals. The role is based at Royal Cornwall Hospital. We have a full programme of training and development which is tailored to meet your individual needs and is supported by experienced and specialist pharmacists. As part of your rotation, you will gain experience in surgery, acute medicine, trauma, emergency medicine, care of the elderly and medicine information.
This is a full-time post working 37.5 hours per week.

Job summary

Job summary
The Breast Services department is strongly supported by a Lead Breast Specialist Nurse and Specialist Nurses in Primary and Secondary Breast Cancer, as well as an Surgical Care Practitioners. There is also a dedicated Breast Clinical Governance and Risk Management Lead.
We offer a wide range of Oncoplastic Breast Surgery, Including Implant based reconstruction, Therapeutic Mammoplasty. Latissimus Dorsi Flap reconstruction, Local artery perforator flap surgery and both wire and magseed guided surgery.
There are weekly countywide MDT meetings for the Breast Cancer, which are well supported by the MDT coordinators, specialist nurses, Histopathology, Radiology and Oncology Consultants.
There are close links between the Plastic Surgery Department at the university Hospital of Coventry and Warwickshire and the other Directorates within the Trust.
Main duties of the job
DUTIES OF THE POST
- Under the guidance of the consultants, the post holder will be responsible for the clinical care of Breast patients.
- There is not expectation of on call
- To assist in Breast Out-patient clinics
- Perform Breast surgery within his/her capability sometimes without supervision and if difficulties arise beyond his/her competence to consult the relevant or on call Consultant.
- To be responsible for the maintenance of proper case records and investigations on patients, and correspondence with General Practitioners and others as required
- Duties will be arranged with the consultant to whom you are responsible, and you must discuss and understand these duties as soon as possible as you take up your appointment. The Consultant has final responsibility for all patients under his/her care and therefore any clinical problems causing concern should be referred to the Consultant, as the senior member of your team, or in his/her absence, the Consultant covering his/her duties.

We are currently looking to recruit a Podiatrist to join the CPFT Podiatry Service working in the community in Cambridgeshire. We have developed a comprehensive preceptorship program for newly qualified Podiatrists, and a structured competency framework for more experienced Podiatrists to ensure those joining the team experience a structured, supportive onboarding experience.
There is scope for the positions to be full or part-time alongside potential for compressed hours (e.g. full-time hours over 4 day weeks or 9 day fortnights).
Main duties of the job
The post-holder will be competent across all core areas of Podiatry and have a strong team work ethic and sense of accountability. The successful candidate will work within the Podiatry team to support our core cohort of patients, with scope for specialist practice dependant on the individuals skillset.

- To be responsible for the assessment and diagnosis of patients with subsequent production of individual treatment plan and be able to manage the day to day running of a busy clinic.
- To complete all relevant internal competency development, including and not limited to; Nail Surgery, MSK, High Risk.
- To participate in the departmental rota for nail surgery as appropriate.
- Assessment and identification of risk factors, categorising patients into appropriate risk banding as defined by the service assessment matrix.
- To be responsible for the day-to-day planning of personal workloads for the completion of home visits and visits to residential homes for the elderly and care centres.
- To be actively involved in the provision of foot health education advice to patients, relatives and carers, to participate in foot health training where appropriate for Primary Care and Social Care staff.
- To be actively involved in the development of departmental policies, guidelines and protocols.
- To be responsible for the maintenance and security of stock and equipment as appropriate.
- To follow departmental policies, standard operating procedures and guidelines using discretion to make own clinical diagnosis and decisions within professional boundaries.
- To be able to treat patients who may be immobile, wheelchair or bed-bound as appropriate.
- To be flexible and provide cover for other Podiatrists at short notice or during periods of absence in other local areas and to undertake any other duties that may be required from time to time.
- To ensure all duties are carried out to the highest standard and in accordance with current quality initiatives within local areas.
- To maintain confidentiality in accordance with the Data Protection Act.
- To carry out all duties with regard to the Trusts Equal Opportunities Policy.
