Clinical Duties:

1. To be responsible for the assessment diagnosis, planning, implementation and evaluation of Podiatric care to a variety of patients with a wide range of clinical needs including:

High Risk Diabetes

Chronic Neuropathy

2. Care will be provided in a range of settings which may include:

The general principles within the Acute & Specialist System are:

Inpatient and consult work

Role summary:

The post-holder will provide general medical servicesand deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Main purpose of the role:

The appointee will have the opportunity to take part in leading on key disease areas (monitoring trends in the Quality and Outcomes Framework, treatment, staff training and prescribing) and assist in developing appropriate healthcare services for a diverse, patient list (encouraging patient participation and health promotion).

Clinical responsibilities:

In accordance with the practice timetable, as agreed, the post-holder will make him/herself available to undertake clinical sessions at either of our two sites (Bransholme Health Centre or Princes Medical Centre), depending on the needs of the practice.

A clinical session is normally composed of face to face appointments, although telephone appointments may be required on occasion. You will have protected time to allow appropriate time for a variety of duties including actioning test results and patient tasks.

When on call the post holder may be required to visit patients at home.

Appointments will be 15 minutes duration as standard.

A student or learner may on occasion be attached to you for a session.

You will be required to:

Act upon most alerts (e.g. QOF) and reminders during a routine consultation.

Provide all GP services required as part of the GMS contract held between James Alexander Family Practice and NHS England, including all core services such as Child Health Surveillance.

Respond to medical problems presented by patients, including history taking,examination, investigation, diagnosis, treatment and referral where appropriate.

KEY RESPONSIBILITIES

Clinical

• Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.

• Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

• As determined by the PCN, support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies.

• Support the Clinical Pharmacist in Structure Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR.

• Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

• Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

• Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

Technical and administration

• Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.

• Implement efficient ordering and return processes and reducing medication wastage.

• Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

• Promotion of Electronic Repeat Dispensing (eRD) and online ordering.

• Develop relationships with other pharmacy professionals and members of the multi-disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health.

• Support practice Reception Teams in streaming general prescription requests, so as to allow GPs and Clinical Pharmacists to review the more clinically complex requests.

• Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.

• Support the PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services.

• Support the PCN in reviewing and developing practice policies for CQC requirements

Job Responsibilities

Clinical Leadership and Governance

• Provide clinical leadership across all aspects of I:HUB.
• Work in partnership with the Operations Manager to ensure the safe, effective and sustainable delivery of the service.
• Serve as the CQC Registered Manager for I:HUB, ensuring ongoing compliance with regulatory requirements and readiness for inspection.
• Lead the clinical governance framework for I:HUB, including audit, incidents, complaints, safeguarding, quality improvement and clinical performance review.
• Lead the development, implementation and review of clinical pathways, protocols and operating procedures.
• Ensure appropriate systems are in place to identify, monitor and mitigate clinical risk.
• Monitor patterns of clinical practice, including referral quality, prescribing, clinical documentation and adherence to agreed pathways, taking appropriate action where improvement is required.
