You would join our long-established nursing team, led by a supportive Practice Nurse Mentor who is motivated to provide a high standard of care in the most effective and efficient way with great emphasis on up-to-date guidelines and standards. Job summary
The post holder is an experienced Practice Nurse, who is responsible for ensuring the delivery of safe and effective patient care to the practice population using autonomous clinical skills and a broad in-depth theoretical knowledge base. Primary Duties & Areas of Responsibility
Clinical Role
- Assess, plan, develop, implement and evaluate treatment programmes for patients that promote health and well-being.
- Help the practice develop and deliver specialist primary nurse led services, including chronic disease management (including for all areas of QOF). Main duties of the job
Your duties will involve the day-to-day responsibility for patients on the assessment wards/Medical SDEC, taking part in consultant ward rounds and ensuring that patient medical case notes are properly completed, drug prescriptions are regularly reviewed, appropriate investigates are carried out and the results acted upon - and made available for consultant ward rounds.
You will ensure that GP electronic discharge letters containing all relevant information are completed and sent in a timely manner, support the teaching of medical students, attend >60% of the formal education events including audit meetings.
You will be allocated an educational supervisor who will provide regular appraisals and be expected to adhere to Trust and Directorate policies and protocol.

Main duties of the job
The Care Navigator plays a vital frontline role within the practice, supporting patients to access the right service, professional, or support at the right time.
The post holder will provide a professional, efficient, and compassionate reception and administrative service. The Care Navigator will support patients accessing services through a variety of channels including telephone, online consultation systems, the NHS App, email and face-to-face contact.
The post holder will be responsible for signposting patients to the most appropriate service based on information provided by the patient and in accordance with agreed care navigation pathways.
The Care Navigator is a non-clinical role. The post holder is not expected to diagnose conditions, make clinical decisions, or provide medical advice.

Main duties of the job
- In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion Including reviewing and actioning lad reports, managing Docman and other clinical correspondence.

Brunel Primary Care Network is seeking an enthusiastic and committed Salaried GP to join our developing Integrated Neighbourhood Care (INT) team, providing proactive, high-quality care to frail, housebound and clinically vulnerable patients across our three practices.

Job summary
We are seeking a friendly enthusiastic salaried GP to join our friendly practice. This position is available due to the reduction in sessions of our long standing senior partner in July 2027 and we wish to give ourselves plenty of time to find the right candidate.
We have a patient list size of 8100 in rural Oxfordshire, Main site at Bloxham with branch surgery at Hook Norton.
We have a full compliment of clinical staff who work closely together to support the practice needs.
Main duties of the job
We do not operate total triage at the practice. Our focus is to accommodate our patients on a daily basis with the options of either a face to face appointment or a telephone consultation depending on which option a patient has chosen.
We have open sessions in the morning with pre-booked appointments in the afternoon.
We have 15 slots in the morning and 12 slots in the afternoon.
You will be supported by a full compliment of nursing to support GP's over both sites. The team consists of 3 nurses, 2 health care assistants and 1 phlebotomist who is trained to do paediatric bloods.
You will be required to work at our branch surgery which is approximately 5 miles away from our main surgery site.
We have 1 private nursing home situated at the end of our driveway for which we cover 1 clinic session per week for their registered clients.
Your contract will be based on the BMA standard contract.

We are currently seeking an experienced GP Plus Practitioner, at Band 6 to work via a Bank Contract, based in Stevenage for our Primary Care Mental Health Service. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.
The role of the GP Plus Practitioner will be to work across the interface between Primary Care and Mental Health Services to work alongside the service manager(s) to facilitate mental health assessment in primary care and support GPs about decision making in relation to secondary care referrals, support transition of service users from the care of secondary mental health services back to Primary Care and to facilitate re-access where appropriate.
The post holder will do this by providing consultation, support, education and problem-solving approaches to the Local Primary Care Teams particularly in the area of assessment and management of mental health problems in adults of working age.
HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving

Main duties of the job
This will include "signposting" to other services and occasionally undertake very short-term interventions for clients with mild to moderate mental health difficulties.
The role is to strengthen access and to provide community triage for people who may need mental health services. The post holder will ensure systems are in place to promote smooth pathways between primary care and secondary mental health services.
Work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.
Work in partnership with Health and social care professionals from other agencies and organisations including Community Navigators
They will demonstrate an understanding of clinical interventions relevant to the Pathway/Protocols to include psycho education, relevant aspects of social inclusion, Recovery, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this.

Are you a compassionate phlebotomist? Our Weymouth and Portland Community Team based at Weymouth Community Hospital are looking for a Community Phlebotomist to join their experienced team.
As the Community Phlebotomist you will support the DN and Anticoagulation teams and housebound patients with their venepuncture requirements. You will be able to work autonomously and manage your own workload (with support). You will be expected to visit patients in their own homes to take required bloods tests and deliver them back to the collection points in a timely manner.
The Community Phlebotomist is a part time role of 24 hours per week, 4 days a week, Monday to Friday. We like to promote a positive work life balance; we can be flexible with how working hours can be met. This includes start and finish times to support childcare/carers responsibilities. Hours can be discussed further at interview stage. In return for your enthusiasm and commitment, we can provide an opportunity for you to develop your clinical skills and experience in a supportive team, with regular supervision and appraisals. We offer full comprehensive induction training with ongoing support and regular supervisions. You must hold a valid UK driving license required and access to a car.

- Undertake Community Phlebotomy for housebound patients in their ownhome who are unable to attend their GP Practice.
- Support nursing team members with delivery of patient care.

Main duties of the job
An exciting opportunity has arisen for a band 3 Health Care Support Worker to work within the Responsive Hospital Support Team, as part of the Older Adult Psychiatric Liaison service. The service is currently based at Nevill Hall Hospital site.
The post will involve working with patients who have both functional and organic mental health diagnosis, in a general hospital setting, under the direction of Band 6 practitioners.
The service works in every general care facility within the ABUHB trust, and applicants will be required to move between sites as part of their role.
The hours of work 8am to 8pm Monday to Friday. 9am to 5pm Bank Holidays and Saturdays/Sundays
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The appointed Pharmacist would be part of a wider pharmacy team working with our associate practices within a Hub model.

Main duties of the job
As the role will develop the successful candidate will take responsibility for areas of Chronic Disease Management and undertake Clinical Medication Reviews, proactively manage patients with complex poly-pharmacy needs. There will also be training to manage low-level investigation results following strict protocol and supervision.
We will look to provide support to develop and train the candidate to further their skills in their clinical interests. The Pharmacist would have an allocated Lead Pharmacist and GP Partner to mentor, support and guide them.
