Job summary
The Patient Services Supervisor(s) will be expected to provide a point of contact for patients and act as a focal point of communication between patients, doctors and other reception staff. They will be responsible for the day to day supervision of the reception team including any ad hoc workers. They will ensure that the deliveries of patient services, within reception, are carried out in accordance with agreed procedures, protocols and time-scales, reporting to the Operations Manager as necessary. They will work opposite each other on a rota alternating between early AM 7:30 - 15:30 and late 11:00 - 19:00 PM shifts.
Main duties of the job
They will lead and motivate the team by setting an example in promoting a professional, positive and efficient image of the practice to all its service users.
Coordinate and manage the workload of the team, delegate duties as needed.
Manage Rota and schedules, including sick leave and annual leave.
Be a point of contact for administrative staff and provide support to the clinical team
They will work opposite each other on a rota alternating between early AM 7:30 - 3:30 and late 11:00 - 19:00 PM shifts.

We currently have 1 Reception Supervisor, 4 Full-time and 1 Part-time Receptionist.
The practice is a 3 GP Partner practice with 9 salaried GP's with 1 Health Care Assistant and 1 Practice Nurse. We also have GP Specialist Trainees

Main duties of the job
You will work as a Primary Care MHP ensuring:
1. Patients with mild to moderate mentalhealth need access the most appropriate treatment for their mental health difficultythrough delivery of rapid and responsive triage, assessment, signposting and brief treatment(1:1, group based and digital platforms).
2. The embedding of mental health expertise in a primary care setting. Working alongside VCS and NMHT colleagues to ensure a holistic, joined up approach for patients with mild-moderate and stable, severe mental health needs, ensuring the most appropriate pathway according to presenting need. You will provide advice and consultation to VCS colleagues in managing issues of complexity and risk, as well as clinical assessment, signposting and brief treatment (1:1 and / or group based) to individuals referred to the clinical element of the service by VCS colleagues within the MDT.

The main clinical duties of this post are in general paediatrics. The Consultant Paediatricians have recently reconfigured their working patterns to participate in a 1:8.5 Consultant of the week system which is split into 2 half weeks, working a block from Monday - Thursday, and a block Friday - Sunday. During the 'hot week' there are 2 Consultants of the week who are both ward based responsible for the ward, COAU, ED emergency calls, and inpatient safeguarding. All consultants also participate in cover on the COAU from 1pm - 10pm leading a team of registrars, junior doctors and Advanced Clinical Practitioners to manage ambulatory patients and review GP and ED referrals. This is covered on a 1:17 rota. Finally all consultants partake in non resident on call. Two consultants are non resident on call at weekends, with 1 consultant on-call overnight.Overall frequency on non resident on calls is1:17. All on-calls and COAU shifts are prospectively covered.

Job summary
This service is a collaboration between Sussex Community NHS Foundation Trust SCFT and the Sussex Federations: Brighton & Hove Federation, Alliance for Better Care and South Downs Health & Care. The Federations will be delivering the GP-led aspect of the Virtual Ward at a place-level to match SCFTs Urgent Community Response service across Sussex. SCFT provides the service in West Sussex, Brighton and Hove and the High Weald, Lewes and Havens area of East Sussex.
B&HF are the lead contractor for the GP Service delivery and have sub-contracted to ABC for delivery within the federations geographical areas of Mid Sussex, Worthing and Chichester.
Since 2022, SCFT have delivered Virtual Wards Hospital at Home in line with the NHS Englands national definition that a virtual ward is a safe and efficient alternative to NHS bedded care that support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home.
As of August 2024, SCFT re-modelled the Virtual Ward delivery and mobilised General Virtual Wards within Urgent Community Response UCR, which supports Sussex patients within its catchment area with a higher level of acuity to be safely managed at home/chosen place of residence. This could be patients on either an admission avoidance pathway or early supported discharge pathway, where the alternative would be NHS bedded care.
Main duties of the job
The Virtual Ward activity is now prioritising admission avoidance whilst still delivering early supported discharge for patients from the Acute hospitals. It is delivered 365 days per year, 8am 8pm. UCR teams within the SCFT footprint provide urgent support to help prevent unnecessary hospital admissions and ultimately keeping patients safely at the place they call home. There is a consistent service across Sussex and is made up of multi-disciplinary teams including General Practitioners, Nurses, Pharmacists, Therapists and Carers, ensuring they can meet a range of needs quickly offering the best level of support to patients.
The multi-disciplinary team that oversees and deliver care the General Virtual Ward caseload is led by both medical a General Practitioner and clinical senior decision makers. Virtual and On Call GP cover is available seven days a week, 9am to 6pm 365 days a year, including bank holidays. This cover is linked to the respective teams local UCR teams with a virtual board round each morning
The UCR Hub Based GP will work within their assigned place based SCFT UCR base. We are recruiting for a GP for the Worthing Virtual Ward. The GP will present within the allocated UCR base working face to face with the SCFT MDT delivering GVW including supervision of ACPs and team development. Patient visits may be required. Monday, Wednesday and Friday morning

This will result in the planning and evaluation of individual care programmes for those individuals.
The post holder will work as part of the multi disciplinary team providing high quality psychological support to patients living with a palliative diagnosis and their carers including some group work.

Main duties of the job
About you
You will have an appropriate professional clinical degree in addition to a relevant well-being or psychological care qualification, you will have experience of providing assessment and support of people presenting with psychological needs and experience of applying an assessment and management process.
Experience in Palliative Care/Oncology and experience of working within a hospice, hospital or similar environment is desirable but not essential.
A full driving licence and access to your own vehicle with the ability to travel throughout South Worcestershire is essential for this role.

Job summary
Two vacancies have opened up in Dalton and Ulverston District nursing team. Various part time hours available. Shift pattern is over 7 days a week and will include regular evening shifts and may be required to cover night shifts to cover sickness and annual leave as the need arises.
The post holders will work as a registered Nurse within Community Services across the Dalton and Ulverston District nursing footprint, accountable for providing evidence based, holistic nursing care of patients.
The post holders will be required to work as part of the integrated team across community settings such as patients' homes, residential and nursing homes. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.

Community nurses deliver a wife range of highly skilled interventions in the patients own home. This includes the management of long term conditions including diabetes care, end of life care, catheter care and symptom control and support. We support patients to remain in their own homes for as long as possible, preventing hospital admission and supporting patients to lead a long and happy life.

Job summary
The Radiology Department at Royal Lancaster Infirmary is looking for an enthusiastic Imaging Support Worker to join our friendly and supportive team.
This is an exciting opportunity to work within a busy diagnostic imaging service, supporting patients and staff across a range of specialist areas including CT, MRI, Ultrasound and the Radiology Day Care Unit.
Applicants should be well organised, have excellent communication and interpersonal skills, and be confident using IT systems. Previous experience within a healthcare environment is desirable, together with a Level 3 NVQ in Health and Social Care (or equivalent experience).
We are looking for a motivated, compassionate and adaptable individual who enjoys working as part of a multidisciplinary team and is committed to delivering high quality patient care.
Main duties of the job
The role involves supporting Radiologists, Radiographers and Radiology Nurses in the delivery of safe and effective patient care across a range of imaging services.
Duties include:
- Supporting patients before, during and after examinations and procedures.
- Assisting with ultrasound, CT, MRI and interventional radiology activities.
- Supporting sterile procedures.
- Undertaking venepuncture and cannulation following appropriate training and competency assessment.
- Chaperoning patients where required.
- Assisting with the smooth day-to-day running of imaging services.
- Undertaking administrative duties including appointment management on the Radiology Information System (RIS) and liaison with wards and clinical teams.
The post is full time, 37.5 hours per week and requires flexible working Monday to Friday on a rotating shift pattern including:
- 07:30 - 15:30
- 08:30 - 17:00
- 12:00 - 20:00
Working days will vary according to the rota and will be provided with a minimum of six weeks' notice. Participation in Bank Holiday working is required and is shared equitably across the team.
