Job summary
An exciting opportunity has presented for a motivated and enthusiastic Care Coordinator to work within a community mental health setting working alongside our primary care colleagues.
The Post holder will support the INT Leads, PCN Manager, Clinical Director and wider PCN team including the PCNs member practices, and multidisciplinary team (MDT) within the PCN with project work as well as general core support to the PCN with a wide range of administration and associated tasks across the network. They will assist coordination of all key activity including access to services, advice, information, and ensuring health and care planning is timely, efficient, and patient-centred.
Our ideal candidate would be a committed team player as well as being able to work independently demonstrating initiative and passionate in promoting good quality care to maximise independence. Within this role you will gain training, support, and guidance to ensure you are working collaboratively with external agencies to give the best possible care for our service users.
Main duties of the job
Responsibilities include but are not limited to the following:
To work closely with the INT Leads, PCN Manager in supporting the Clinical Director, PCN practices and team in delivering the PCN Contract DES Specifications. The successful candidate will need to understand the importance of engaging with external agencies within our area in order for our service users to achieve high-quality care that we in Navigo always strive to deliver. This should involve using supervision and reflecting on practice to develop new and improved ways of working.
Undertaking a wide range of administrative duties and providing administrative support to the Primary Care Network and team, including arranging MDT meetings, minute taking and recording.
Assisting the INT leads & PCN Manager with their relationships and engagement with the PCN practices and other stakeholders across the locality to ensure a collaborative approach to service development and delivery.
Build and maintain PCN clinics on Systmone along with booking and cancelling patient appointments.
Utilising population health intelligence to proactively identify and work with cohorts of patients across the PCN
Have a proactive approach, the ability to work on your own initiative and within a team environment.
This role will require the post holder to be based at Jubilee Gardens Medical centre and have the ability to visit PCN practices if and when required.
About us
Northolt Primary Care Network started in the year 2019. We have 6 GP practices in the PCN. You will be working with 1 or more practices within the PCN.

Job summary
This role sits at the heart of an exciting strategic partnership between KMMH, and a number of GP practices in Kent & Medway, established to embed research capacity within community mental health services across east Kent and Swale. These GP practices deliver the Primary Care Mental Health Service (PCMHS), supporting over 5,000 people annually, many from coastal communities that are among England's most deprived and historically under-represented in research.
The post holder will lead the delivery of clinical research studies across these primary care settings, working 0.8 WTE shared across practices. You will be responsible for the identification, recruitment, delivery and follow-up of research participants in accordance with study protocols, ensuring research is conducted safely, ethically and to the highest standards. Working under the mentorship and supervision of the KMMH Research Development Lead & Research Delivery Leads, you will help build the research capability and governance infrastructure needed to make these GP practices research-active, sustainable partners and help us live our values every day.
Please note there will be regular travel between GP sites, so a driver would be preferred.

o Work with GP practice leadership to incorporate research governance into quality assurance frameworks and operational policies, embedding research into business-as-usual activity.
o Contribute to the development and approval of site-level SOPs and readiness processes to achieve research-ready status within participating practices.

Victoria Road Surgery in Lowestoft was founded in the 1940s and currently serves just under 10,500 patients. There are six GP Partners who own the business which works under various NHS contracts. The Partners employ nurse practitioners, practice nurses, healthcare assistants and administrative staff including receptionists, secretaries and managers.
Victoria Road Surgery works in partnership with other Lowestoft practices as "Lowestoft Primary Care Network" and benefits from PCN staff including a pharmacy technician, health & wellbeing coach and a care-coordinator who are based at the surgery. We have a vacancy for a full-time Medical Secretary to work between Charlton Lane and Wotton Lawn Hospitals within an established team. The post is fixed term for a period of 12 months, working Monday to Friday 37.5 hours each week.
Relevant secretarial experience is essential as the environment is fast-paced requiring a broad knowledge of medical terminology, RSA II/OCR II Word Processing/Typing and audio typing experience. Job summary
Betsi Cadwaladr University Health Board wishes to appoint two full time Locum Consultant Dermatologists to be based at either Wrexham Maelor Hospital or Glan Clwyd Hospital.
The posts are for ten sessions per week of which,eight sessions will be for Direct Clinical Care (DCC) and two for Supporting Professional Activity (SPA). Part time/ flexible working may be considered following discussion and agreement with the Clinical Lead and Divisional Medical Director.
There is an expectation to see a case mix of adult general dermatology and skin cancer in line with service needs.
The departments currently run Teledermatology clinics, and suitable candidates may be able to undertake Teledermatology clinics as part of direct clinical care programmed activities in line with service needs.
Main duties of the job
To provide dermatology outpatient services to patients in collaboration with nursing and medical colleagues.
To provide dermatological inpatient care and provide specialist advice for inpatients as part of acute dermatology cover.
To share responsibility for professional standards and quality control within the dermatology services ensuring compliance with standards which satisfy clinical governance.
To develop dermatological service with other consultant colleagues in recognition of regional and national guidelines where appropriate.
To share in providing emergency cover for colleague's study/annual leave or illness.
To develop and implement clinical audit programmes within the Dermatology Departments and in collaboration with other clinical groups.
To participate in consultant annual appraisal.
To participate in service development and business planning in collaboration with other consultant colleagues and the wider health community.

Main duties of the job
The job plan is anticipated to contain 10 programmed activities with an approximate split of 8.5 direct clinical care programmed activities (CPA) of Diabetes and Endocrinology and General Internal Medicine and 1.5 PA's for supporting professional activity (SPA). However, we are willing to accommodate any different number of PAs as the service permits.
Alongside colleagues, you will provide specialist led inpatient and outpatient Diabetes & Endocrine services. This includes ward referrals and supporting the team of trained DSNs.
Your prime clinical responsibility will be accountability for the continuous care of in-patients and outpatients, and as part of this responsibility, a share of the out-of-hours general medical on -call rota arrangements at the Trust.
You will work as an integral part of the Multidisciplinary Trust wide Diabetes network and attend Clinical Governance and Multidisciplinary Team meetings at multiple Trust and external provider sites.
Appropriate standards of care for patients must be a priority as well as appropriate standards of education and training for medical staff in training. Therefore, you will contribute to the teaching of medical students, trainee advanced care practitioners, specialist nurses and participate in weekly postgraduate meetings as well as audit, governance, morbidity and mortality and training meetings.

Main Tasks Duties and Responsibilities Clinical Role
Monitor, interpret and evaluate patient test results and initiate appropriate treatment, or refer on if required
Make appropriate referrals to primary, community and secondary health care teams
Systematically manage chronic disease in the patient population, including asthma, diabetes and COPD
Undertake opportunistic screening when clinically required e.g. lipids, blood pressure, cervical smears, etc.
Perform venepuncture and electrocardiography
Undertake wound care / management, dressings and suture removal
Undertake leg ulcer management, dopplers and compression bandaging
Carry out ear examination and micro suction
Perform cervical screening and promote breast cancer awareness
Provide an adult and paediatric immunisation service under individualised prescriptions or Patient Group Directions, ensuring safe storage, rotation and disposal of vaccines and drugs
Provide other injections as required and within competence, both IM and SC
Provide advice on lifestyle issues, assist and support patients in changing their behaviours; refer appropriately.
Deliver the smoking cessation service
Deliver travel health care, including vaccinations and travel advice
Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and be aware of local referral procedures
Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries
Undertake first aid and management of emergencies, e.g. burns, haemorrhage, hypo/hyperglycaemic emergencies/asthma attacks
Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques
Provide contraception and family planning advice, including pill checks
Undertake new patient and NHS health checks
Support the practice in achieving quality indicators and targets of the GMS contract and any other relevant service frameworks

Apply infection control measures within the practice according to local and national guidelines, including:

Job summary
We are looking for an experienced dietitian to provide maternity cover within a Primary Care Network in South Haverings. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices.
As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community.
This is a temporary contract until 31st December 2026.
Main duties of the job
You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team. Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, frailty and paediatrics - helping them make lasting changes through person-centred care and behaviour change approaches.
You will also have opportunities to contribute to service improvement, clinical audit, and pathway development. Group education sessions may form part of the role depending on service needs.
