• Performing highly skilled physiotherapeutic assessment of patients referred by Consultants, General Practitioners and other Healthcare Professionals with diverse presentations and complex and acute/chronic physical conditions and sports injuries. To use clinical reasoning skills to provide an accurate clinical diagnosis.
• Communicate effectively with patients to ensure informed consent for assessment and treatment, good understanding of their conditions and maximise their rehabilitation potential.
• Managing and prioritising own clinical caseload efficiently and effectively.
• Formulate, develop and deliver a specialised individualised treatment programme based upon evidence based practice, manual assessment and appropriate clinical reasoning.

The Lead Practice Nurse will provide clinical leadership and professional support to the practice nursing team, ensuring the delivery of safe, effective and high-quality nursing care.

The post holder will maintain an appropriate clinical caseload and work autonomously within their professional scope of practice. Job Summary

The post holder will work as part of the practice team, responsible for the accurate and timely summarising and clinical coding of patient correspondence. Scope and Purpose of Job Description

Job Purpose

The post holder will help patients to get the nest from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake clinical sessions. A clinical session is normally composed of 16 face to face appointments with a minimum of 2 telephone appointments, followed by a variety of duties including queries, visiting patients at home, checking and signing repeat prescriptions, actioning test results, medical reports and examinations (e.g. Insurance Companies) on behalf of the Practice. The above duties should be performed in a timely manner. Appointments will be 10 minutes duration and telephone appointments will be of 10 minutes duration. A student or learner maybe attached to you for this session.

You will be required to:

• Act upon most alerts (e.g. QOF) and reminders during a routine consultation i.e. (10 minutes).
• Provide all GP services required as part of the GMS contract held between Library House Surgery and the commissioning CCG, including all core services.
• Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate.
