We have an opportunity to join our growing team at The Homewell Practice as a Care Coordinator-Navigator. Our practice has expanded significantly over the last four years, and with the increasing demand for GP services, we are strengthening our team to continue delivering high quality patient care.

We are looking to recruit a Care Coordinator-Navigator to work 37.5 hours per week, joining our supportive and friendly practice family.

You MUST be able to work until 6.30pm on Tuesday, Wednesday, Thursday and Friday.

We are looking for someone who is flexible, reliable, and committed, someone who sees this as a long term career opportunity. This role suits a person who enjoys learning, thrives in a busy environment, and takes pride in helping patients access the right care at the right time.

Experience in a busy customer facing role is essential, and experience within a healthcare setting is desirable. This is a varied and fast paced role where no two days are the same. It is not a role that can be learned overnight, so we are seeking someone who is motivated by the challenge and eager to grow. Full training will be provided.

Main duties of the job

The post holder will support the smooth running of the practice by coordinating patient care, navigating individuals to the most appropriate services, and ensuring effective communication across the healthcare system.

You will present a positive, friendly, and professional image to patients and visitors, whether in person or over the phone. You will help patients access the right clinician or service in a courteous, efficient, and effective manner, while supporting communication between patients, the primary care team, secondary care, and other healthcare agencies.

Key responsibilities include (but are not limited to):

Handling incoming calls from patients and healthcare agencies

Supporting patients to access the right service, clinician, or pathway

Managing patient administration, including registrations, deductions, and demographic updates

Processing clinical and administrative information from secondary care and other providers

Completing administrative tasks as directed by clinicians and the leadership team

Main duties of the job

The job plan is anticipated to contain 10 programmed activities with an approximate split of 8.5 direct clinical care programmed activities (CPA) of Diabetes and Endocrinology and General Internal Medicine and 1.5 PA's for supporting professional activity (SPA). However, we are willing to accommodate any different number of PAs as the service permits.

Alongside colleagues, you will provide specialist led inpatient and outpatient Diabetes & Endocrine services. This includes ward referrals and supporting the team of trained DSNs.

Your prime clinical responsibility will be accountability for the continuous care of in-patients and outpatients, and as part of this responsibility, a share of the out-of-hours general medical on -call rota arrangements at the Trust.

You will work as an integral part of the Multidisciplinary Trust wide Diabetes network and attend Clinical Governance and Multidisciplinary Team meetings at multiple Trust and external provider sites.

Appropriate standards of care for patients must be a priority as well as appropriate standards of education and training for medical staff in training. Therefore, you will contribute to the teaching of medical students, trainee advanced care practitioners, specialist nurses and participate in weekly postgraduate meetings as well as audit, governance, morbidity and mortality and training meetings.

Job summary

We are looking for an organised, efficient and friendly patient administrator for this newly designed post. The postholder will be responsible for clearing the task list in SystmOne, ringing and booking patients in for reviews and tests, and responding to queries. They will work alongside the care navigation team and clinical teams to help deliver an excellent standard of care for our patients.

The post is 24-28 hours per week, with days and hours to suit the successful candidate.

A GP practice is a fast-paced, teamworking environment, requiring the ability to work under pressure and to absorb regular changes. Excellent attention to detail and high levels of accuracy are required to ensure an excellent and safe service for our patients.

Main duties of the job

Plan, perform and interpret quality-assured diagnostic and monitoring spirometry (including bronchodilator reversibility testing where indicated) to support the diagnosis and ongoing monitoring of COPD, to ARTP / national spirometry quality-assurance standards.
