Job summary
The main purpose of the job is to provide reception and administrative support to the GP Practice to enable the medical and nursing staff to provide an effective, efficient, and caring service to the practice population.
The reception area at the surgery is currently open 8:00am until 6:30pm Monday to Friday each week and has a late surgery until 8:00pm hours on a Tuesday. The medical reception staff have various duties which include manning the reception desk, dealing with queries from both patients and outside agencies and other administration tasks.
The post is essential for the smooth running of the practice and excellent customer service skills are required.
Main duties of the job
- Greet patients and book them into the computer system on arrival.
- Direct patients to the waiting room.
- Taking in prescription requests whilst on reception.
- Notify patients when clinics are running late.
- Answer the phones & deal with all enquiries.
- Book and amend appointments.
- Deal with enquiries from other healthcare professionals and other agencies as requested in an efficient manner.
- Completion of forms as requested.
- Extract notes and patients' summaries from computer.
- Register new patients following the practice protocol.
- Administration tasks such as completing registrations, deductions, booking interpreters.
- To bring to the attention of the Practice Management team all complaints, comments, and suggestions.
- To report any adverse/untoward incidents to the Team Leader.
- To ensure that any accidents/ or near misses are recorded in the accident book and relayed to the Team Leader.
- Dealing with emergency calls or taking requests for urgent house calls and notifying the Team Leader immediately.
- Attend to patients when an emergency arises in the practice & notify Doctor/Nurse immediately.
- Completing a morning and evening checklist such as adding the clinicians' names to their room doors, replacing bed rolls, and turning computers on/ off.
- Assist in the induction of a new member of staff/or student.

Job summary
We are seeking a reliable and enthusiastic Patient Adviser to join our friendly and supportive team at Portland Medical Centre, part of the Bourne Health Partnership. The Patient Adviser team is the first point of contact for patients and all visitors at Portland Medical Centre. The Patient Advisers provide a reception service and handle all queries from patients on reception. The team also deal with all day-to-day enquires that come to the practice via our online portal.

Main duties of the job
The Diabetes Practice Nurse will perform tasks prescribed and delegated by the GP Partners, working unsupervised to PGDs, agreed protocols and within defined competencies.
The Diabetes Practice Nurse will assess patients and plan, implements and review treatment programs. The Duabetes Practice Nurse will have shared responsibility for training and supervision of new members of the nursing team and colleagues.

With previous experience from the NHS or private care, you will oversee and provide high-quality care and support to people in a Neurological setting, while supporting your team and colleagues and promote good teamwork. What you will be doing
With previous experience from the NHS or private care, you will oversee and provide high-quality care and support to people in a Neurological setting, while supporting your team and colleagues and promote good teamwork. As a Charge Nurse, you’ll lead and coordinate the quality and management of nursing care and delivering of clinical services, ensuring the day-to-day operations of your unit is performing well. You'll also;
- Delegate nursing tasks to Nurses working in the unit and coordinate with other teams
- Monitor service users’ conditions and respond to changes
- Supervise other members within the team's unit and provide guidance and support

We have a vacancy at West Leeds PCN for a prescribing Nurse, Paramedic or Nurse Practitioner. This role would particularly suit an Advanced Practitioner with good all round nursing experience as the role itself will be very broad. We need someone who is an experienced Independent Prescriber (minimum of 3 years experience), particularly for the Acute Home Visiting element of this role. You would be joining a highly skilled and experienced team with a wealth of peer support available each day.
Main duties of the job
There are two elements to this role:
Annual frailty reviews - these are templated and in depth. We focus mainly on those that are severely frail but we also get the opportunity to work with the moderately frail, changing their health outcome in good time to avoid them from tipping into the severely frail Rockwood score (which you will find immensely satisfying). These are mainly done in the patient's home unless they are able to get into surgery
Acute home visits - you will spend a part of your working week doing these as this services relieves a great deal of pressure on our member practices (particularly the GPs)

The intention is to either improve or maintain their level of health and wellbeing and also to enable them to remain as independent as possible in their own residence. The Frailty Team does not carry a caseload; each referral is classed as an intervention. Following review from the Frailty Team the patient care is handed back to their GP practice.
Each patient is assessed for falls prevalence and has their medications reviewed. An independent prescriber would be an advantage for this role but it is not essential as other members of the team can issue medication
You will attend weekly MDT meetings with a consultant geriatrician hosted by our frailty team to discuss the most complex patients who have regular home visits. Consultant in Gastroenterology WSI Inflammatory Bowel Disease
The Inflammatory Bowel Disease (IBD) team are looking to recruit a substantive Consultant Gastroenterologist with a specialist interest in IBD to join our dedicated and highly motivated team. The post holder will join our current five Consultant IBD physicians and will be an active member of the multi-disciplinary team.
The successful candidate will work across three hospital sites within the Trust including Sandwell Health Campus, City Health Campus and our new state of the art acute care hospital facility, Midland Metropolitan University Hospital (MMUH).
Applicants must be a member of the Royal College of Physicians (MRCP) or equivalent. Completion of higher specialty training in Gastroenterology or comparable is essential. Candidates applying for this post must either be on the Specialist Register for Gastroenterology or be within six months of being on the Register for trainees currently in a training programme in the UK.

Main duties of the job
The post will enable the successful applicant to develop sub-specialty interests in inflammatory bowel disease to complement the existing consultant staff.
The appointee will take equal responsibility with existing Consultants for the proper functioning of the clinical service, working with clinical and managerial colleagues towards achieving and maintaining national maximum waiting times and developing clinical pathways in line with the maximum 18-week patient pathway guarantee. There will also be a commitment to the gastroenterology ward on-call as part of this post (consultant of the week 1, COW1), which at present is 1 in 12 (which may change in future) providing gastroenterology input 7 days a week and currently has a supplement of 3%. There will be a rest day following the COW1 week on Monday. The successful candidate will also undertake a consultant of the week 2 (COW2) shift twice a month providing in reach gastroenterology service to the acute medical unit in the morning followed by an inpatient endoscopy list in the afternoon. They will also take part in advice and guidance from primary care as well as doing the gastroenterology and triage service virtual clinic of 20 GP referrals per clinic. The appointee will be required to undertake 2 weekend endoscopy lists each year as part of their job plan.

Job summary
We are part of Suffolk Primary Care, which is a group of GP practices delivering patient care across Suffolk. We are seeking to recruit an enthusiastic and flexible salaried GP, with excellent interpersonal skills, to join our fantastic team at Derby Road Surgery in Ipswich.
The vacancy is for 6 sessions per week, across a Monday (2), Tuesday (2) and 2 sessions to be completed outside of core hours each week; these out of hours sessions will be paid at an enhanced rate of pay. Flexibility is crucial to accommodate training, holidays and occasional sickness.
Derby Road Surgery supports in excess of 19,000 patients from a diverse range of backgrounds. We have an established team with a strong focus on customer service and on developing and nurturing staff in order to deliver excellent standards of patient care. We use SystmOne and askmyGP to facilitate effective and efficient ways of working.
You should be able to work calmly and professionally as part of a team, with the ability to multi-task. A strong command of written and spoken English with an excellent telephone manner is a must. Good time management skills and the ability to follow protocols and or instruction and attention to detail is essential.
