The Practice Nurse will provide high quality nursing care as part of our dedicated nursing team, working with the wider practice to deliver outstanding patient care. The main focus will be on general nursing duties such as dressings and immunisations as well as ensuring there is efficient stock for upcoming appointments. The Practice nurse will also play a crucial role in the delivery of our flu and Covid vaccines.

The service is made up of speech and language therapists, assistants and administrative staff.

The service promotes speech, language and communication development and offers Early Help, providing more advice and help at an early stage, working with system partners. This involves delivering training to early years settings and schools and acting in a consultative capacity to manage requests for help. Specialist services are then offered as required.

The service has undergone significant re-design over recent years and delivers services in line with the iThrive model, which puts prevention at the heart of everything we do. • Provide high-quality and advanced care for patients of all ages with a range of conditions, including acute and chronic illnesses. They perform physical exams, take medical histories, and order diagnostic tests. They also develop and implement care plans, prescribe medications, and manage ongoing care via face to face, video or telephone consultations.
• Make clinical decisions based on diagnostic results and adjust treatment plans as needed. They must demonstrate safe clinical decision-making and expert care. And work within own scope of practice.
• Work with physicians, specialists, and other healthcare professionals to provide patient-centered care. Provide clinical triage of patients to appropriate services and healthcare professionals.
• Educate patients and their families about healthy lifestyle choices, disease prevention and self-management techniques.
• Stay up to date with the latest evidence and standards of care through ongoing education and research.
• Mentor less experienced nurses and staff.
• Support the Practices future development and share the Practices values.
• Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
• Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
• Provide wound care where appropriate
• Provide specialist clinics such as contraception where appropriate
• Undertake the collection of pathological specimens
• Request pathology services as necessary
• Process and interpret pathology and other test results as required
• Maintain accurate clinical records in conjunction with extant legislation
• Ensure SNOMED CT codes are used effectively
• Be responsible for the implementation of an effective immunisation programme
• Chaperone patients where necessary
• Prioritise health issues and intervene appropriately
• Support the team in dealing with clinical emergencies
• Recognise, assess and refer patients presenting with mental health needs
• Be an extended and supplementary prescriber, adhering to extant guidance
• Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
• Contribute to practice targets (QOF, etc.), complying with local and regional guidance
• Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
• Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
• Support the clinical team with all safeguarding matters in accordance with local and national policies
• Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyProvide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
• Deliver opportunistic health promotion where appropriate

Secondary Key Responsibilities:

In addition to the primary responsibilities, the Advanced Nurse Practitioner/Advanced Clinical Practitioner may be requested to:

• Act as the audit lead, effectively utilising the audit cycle
• Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
• Monitor and ensure the safe storage, rotation and disposal of medicaments
• Participate in local initiatives to enhance service delivery and patient care
• Support and participate in shared learning within the organisation
• Continually review clinical practices, responding to national policies and initiatives where appropriate
• Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)
• Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation
