Job summary

Ealing Community Partners / West London NHS Trust are seeking to recruit professional permanent band 3 team administrators to join the Ealing Community Partners Referral Hub.

The referral hub has about 30 staff, both clinicians and administrators, working around the clock to facilitate both urgent and routine pathways to in-excess-of 85,000 referrals per year for the majority of the Community Health Care services within Ealing.

The role will require you to work closely with other administrators and clinical staff across the ECP localities, GP surgeries, team leads, patients and their relatives, while handling sensitive personal data.

The ideal candidates will have excellent written and verbal communication, a warm telephone manner, strong administration skills and good IT knowledge, with the ability to work flexibly within a team in a challenging environment of a 24 hour 7 days a week office.

Main duties of the job

The role is varied and duties include:

Checking patient eligibility.

Registering all referrals on the SystmOne IT platform.

Maintaining and reporting on service-specific waiting lists.

Requesting clinical triages when indicated.

Sending appointments directly by letter or arranging on the telephone after patient responses to opt in texts/letters.

Answering patient enquiry telephone calls; giving basic information when able, or transferring the call to the specific service treating the patient or escalating to team management.

Receiving and recording accurate and concise messages, ensuring messages are forwarded on appropriately and timely.

An exciting opportunity has arisen as part of this model for a registered nurse with a keen interest in providing outstanding support within mental health community settings.

This is an exciting new role that builds upon the success of other Additional Roles Reimbursement Scheme posts (ARRS) in physical health to create mental health specific roles that work alongside our GPs and other Primary Care colleagues, while receiving clinical support from our multidisciplinary mental health teams in West London Trust.

The role will offer mental health assessments and brief interventions to referred patients and will also work to support the development of mental health expertise within a defined group of GP practices aligned to a specific PCN.

As one of our MHARRS practitioners you will be employed on a permanent NHS contract and will be hosted within a defined PCN. You will work alongside GPs, practice nurses, social prescribers and other primary care based professionals to help deliver an integrated and holistic approach to care.

Throughout your employment in this role you will receive regular supervision from a Senior clinician within MINT and will work within an agreed scope of practice, although we expect this to change and develop as the role develops.

Main duties of the job

The post holders will provide assessments, formulation and delivery of the psychological intervention/s in primary care, as well as supporting access to MINT or other services such as Early Intervention in Psychosis (EIP), Improved Access to Psychological Therapies (IAPT where needs can be better met by these services.

The post holder would be expected to work closely with experts by experience, local communities and will also be involved in raising the awareness of mental health difficulties in the general population.

o Review and support the practice lists of SMI including completion of annual reviews with the GPs.

o Where appropriate; proactively in conjunction with GPs identify and clarify where people would benefit from mental health input (e.g. IAT or other services such as MINT)

Thank you for your interest in this post for as a Care Home Practice Foundation Pharmacist in the Care Home GP Practice.

The Care Home GP Practice is part of Ealing Community Partners (ECP) and provides an enhanced primary care service to Care Homes (Nursing and Residential) across Ealing. The team includes GPs, Pharmacists, Nurses, Advanced Practitioners and Administrators and the service is provided 8am - 8pm 7 days per week.

The service works closely with colleagues across ECP and Ealing's Borough Based Partnership teams as well as Care Home providers with the aim of providing an integrated approach to the care available to the residents.

The service has transferred to the Trust, from a previous subcontracted arrangement, and this will give the opportunity to explore the current and future delivery of the service via a transformation programme.

Main duties of the job

The ECP Care Home GP Practice provides enhanced primary care services for over 1000 patients in nursing homes and care homes in the borough of Ealing.

The post holder is a pharmacist, who acts within their professional boundaries, working as part of a multi-disciplinary team in a patient-facing role. The post holder will assist in areas of patient care and chronic disease management within the care home practice, undertaking clinical medication reviews to manage patients especially for polypharmacy, older people, residential and nursing home residents with multiple co-morbidities.

The post holder will provide support to pharmacy team and care home general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing advice on clinical medicines issues while addressing both public and social care needs of patients in primary care.

The post holder will provide clinical advice on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

Job summary

The Trust is seeking a consultant forensic psychiatrist to join Broadmoor Hospital. The vacancy has arisen as the result of another consultant leaving. The successful applicant will be the Responsible Clinician for 15 patients on an increased support and assertive treatment (ISAT) ward. Main duties of the job

The Post Holder will be responsible for the care of 15 patients on an increased support and assertive treatment (ISAT) ward. Patients on the ISAT wards will generally require more intensive support and more likely to be displaying behavioural disturbance. The Post Holder will work as part of a multidisciplinary team, consisting of a ward manager, ward nursing staff, a clinical or forensic psychologist, a resident doctor, a social worker, a security liaison nurse and an occupational therapist. The Post Holder will be expected to provide leadership to the Clinical Team and co-ordination of the Care Plan in relation to each individual patient within the framework of the Care Programme Approach.

As part of the Clinical Team standards within the hospital, it is expected that the Post Holder will see his or her ISAT patients regularly, in addition to Clinical Team Meetings.

The Post Holder will be responsible for carrying out administrative duties in relation to the care of his/her patients including completion of reports to the First-Tier Tribunal (Mental Health), Annual Statutory Reports to the Ministry of Justice, and other statutory and non-statutory reports within the required timescales. Ealing NHS Talking Therapies was one of the first Talking Therapies services to launch in 2008, and is one of the three services provided by West London NHS Trust. We are a large team of over 70 members of staff and are currently working on further expansion. Our main base is in Ealing where we have access to over 16 clinic rooms and two open plan offices. In addition to this, we run clinics at GP surgeries across the borough, covering areas such as Southall, Acton, and Chiswick. Ealing Talking Therapies is passionate about offering a great quality therapy to our patients and we are very proud to be able to consistently offer all the Talking Therapies-approved therapies, such as EMDR, DIT, IPT, BCT, and CfD. CNWL is looking an enthusiastic and highly motivated Consultant to join the Harness Community Mental health Team.

This full time position would be excellent for someone wanting to broaden their experience of community psychiatry and develop their clinical skills in a supportive and challenging environment. The team is well established with good support from senior clinical and managerial colleagues.

Main duties of the job

This is a Consultant Psychiatrist replacement post working within the Harness Locality Community Mental Health Team. The post holder will have consultant and RC responsibility for a defined caseload of service users under the care of the local CMHT. The post holder will also provide mentoring and supervision as an experienced consultant.

Job responsibilities

To manage, appraise and provide professional supervision to junior medical staff as agreed between consultant colleagues and the medical director.

To ensure that junior medical staff working with the post holder, operate within the parameters of the New Deal and are Working Time Directive compliant.

To record clinical activity accurately and comprehensively, and submit this promptly through the trusts clinical information systems.

To participate in service and business planning activity for the locality and, as appropriate, for the whole mental health service.

To participate in annual appraisal for consultants.

To attend and participate in the academic program of the Trust, including lectures and seminars as part of the internal CPD program.

To participate annually, as a minimum, in a job plan review with the clinical manager, which will include consultation with a relevant manager in order to ensure that the post is developed to take into account changes in service configuration and delivery associated with modernization.

To work with local managers and professional colleagues in ensuring the efficient running of services, and share with consultant colleagues in the medical contribution to management.

To comply with the Trusts agreed policies, procedures, standing orders and financial instructions, and to take an active role in the financial management of the service and support the medical director and other managers in preparing plans for services

Assessment of new patients and clinical supervision of assessments carried out by other team members.

Full participation in the multidisciplinary team providing care in the community.

Senior medical responsibility for the care carried out by the Community Mental Health Team for both CPA and LPC patients.

Review and monitoring of the caseload to ensure appropriate care packages are put in place and discharges facilitated to maintain patient flow.

Contribution to the continuous development of the Community Mental Health Team, and to other parts of the Service as required.

Domiciliary visits to provide assessment and treatment services as a member of the CMHT

Domiciliary assessments in response to emergencies

Research, in terms of personal projects and clinical audits as part of clinical governance.

Involvement in joint planning of future service development.

Occasional psychiatric assessments of residents who may be in crisis in another borough.

Close liaison with the wards to facilitate admission and discharge planning.

Liaison with HTT, Psychiatric Liaison Services, AMHP services and Forensic Services in the borough.
