Job summary
Job summary
Abington Park Surgery is looking for a caring, confident and motivated Salaried GP to join our innovative, well-established team (4 sessions per week Monday s and Wednesdays ). Log term locum are ideal. Both experienced and newly qualified GPs are welcome to apply. We cannot support with a Tier 2 visa for this short term role.
- In accordance with the current access model, to undertake a variety of duties, including duty days, surgery consultations, telephone consultations, visiting patients at home, on the day duty, checking and signing repeat prescriptions and dealing with queries, admin paperwork and correspondence in a timely fashion
- Demonstrate Leadership and effective management of the practice and actively participate in the service improvement
- Active contributor to improving practice performance
- Actively participate in the PCN activities
- Be a keen learner and develop special clinical interests in QOF management
The Package
- 4 sessions per week Monday and Wednesday
- Competitive salary, depending on experience, upto 11K per session annually FT or PT pro-rata

Main duties of the job
- The delivery of high quality medical care to the practice population
- Making professional, autonomous decisions in relation to presenting problems, assessing the health care needs of patients with undifferentiated and undiagnosed problems,
- Screening patients for disease risk factors and early signs of illness, developing care plans for health in consultation with patients and in line with current practice disease management protocols
- Recording clear and contemporaneous consultation notes to agreed standards
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
- Processing of administration in a timely manner, including your own referrals, repeat prescription requests, documentation and other associated administrative tasks
- Show leadership qualities, working collaboratively, ensuring an even distribution of the practice workload and be a team player. Accept delegated responsibility for a specific area (or areas) of the QOF and IIF. Effectively contribute to the practice performance
- Identify, implement and embed best practice through clinical guidelines and the audit process, Commit to self-learning and support an ethos of continuing professional development across the practice team
- Contribute to the training of medical students and GP Registrars
- Encourage collaborative working, liaising with all staff regularly, promoting a culture of continuous improvement at all time

- We are one of Northamptonshire' s leading training practices, consisting of six Partner GPs, six Practice Nurses, a fabulous support team and nearly 13000 patients. - We use Accurx triage, scribe, Systm1 clinical systems.

- Providing advice on the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
- Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients.
- Management of the full range of infection control procedures in both routine and extraordinary circumstances (e.g. pandemic or individual infectious circumstances)
- Hand hygiene standards for self and others
- Managing directly all incidents of accidental exposure

Cruddas Park Surgery are excited to recruit a Salaried GP for six sessions including one fellowship session (negotiable/pro-rata). The post would offer a new challenge to an established GP or the opportunity to flourish as a newly qualified GP.
Cruddas Park Surgery believe we offer a unique approach to Deep End primary care supporting the most vulnerable and invisible through our motivated and expert clinical and non-clinical staff, specialist clinics (including Health Inclusion drop ins, Learning Disability and Special Allocation Scheme), links with the VCSE and Local Authority, and innovative approaches to tackling mental illness through The Space, our multi-agency one stop mental health hub.
The fellowship session has been a successful model in the past and would include a mutually agreed period of protected non-clinical time and regular senior supervision on joining the practice. This session would then become an opportunity to develop an area of clinical or non-clinical interest which was of benefit to our patients.
Employment is subject to the terms of the BMA contract at £12,925 per session including paid CPD time and 6 weeks holiday pro rata. Current routine sessions are 11 new patient face to face appointments. A shared practice electric car is available for home visiting. We have a clear desk policy for holidays and active social calendar including a running club.
Main duties of the job
The post-holder will manage an unselected caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients in line with local and national guidelines as well as practice policies. They would be expected to contribute to the pro-rata Duty and Triage rota and home visits; assist in education, supervision and training; as well as play a full role in achieving local and national targets and quality improvement targets. Job summary
We are seeking an experienced Practice Nurse to work alongside our clinical team to play an important role complementing and enhancing the Rise Park Surgery team. This is a part time role of 20 hours per week.

The post holder is responsible for the delivery of practice nursing services care to the practice population. Job summary
Bulwell and Top Valley PCN (Lead Employer: Rise Park Surgery) is recruiting an enthusiastic and highly organised Extended Access & PCN Services Coordinator to support the delivery and ongoing development of services across our network.
This is an exciting opportunity for an individual with excellent administrative, organisational and communication skills to play a key role in coordinating our Extended Access Hub and supporting the effective delivery of PCN services. Working closely with GP practices, clinicians, PCN staff and external stakeholders, the successful candidate will ensure services are delivered efficiently, appointments are managed effectively, and performance data and reporting requirements are maintained to a high standard.

Main duties of the job
The Extended Access & PCN Services Coordinator will support the delivery of high-quality services across Bulwell and Top Valley Primary Care Network. Main duties of the job
As PCN Manager, you will provide operational leadership across the AiC Primary Care Network which operates as a Limited Company, working closely with the Clinical Director, PCN Board and member practices to deliver the Network Contract DES and achieve the PCN's strategic objectives.
You will lead on the day-to-day management of the PCN, supporting workforce planning, financial management, service development, project delivery, governance and contractual requirements. The role also involves working collaboratively with GP practices, the Integrated Care Board (ICB), neighbourhood teams and wider health and care partners to strengthen integrated working and support the development of local neighbourhood services.
This is a varied and rewarding role requiring excellent organisational, communication and problem-solving skills, with the opportunity to lead service improvements, manage multiple workstreams and contribute to the future transformation of primary care, ensuring the best possible outcomes for our patients and local communities.

Eden Park Surgery is looking for a reliable and organised Prescription Clerk to join our team as part of a job share arrangement. This is an important administrative role supporting the safe, accurate and timely processing of prescription requests for our patients.
The successful candidate will work closely with the existing Prescription Clerk, pharmacy team, clinicians and wider practice team to help ensure medication requests are managed efficiently and in line with practice protocols.

Principal Duties and Responsibilities
- Medication Reviews:
Conduct structured medication reviews with patients with polypharmacy, frailty and long term conditions.
Support the repeat prescribing process, ensuring medicines are prescribed safely and cost-effectively following best practice guidelines.
Provide advice and clinical support on prescribing and medicines optimisation to GPs and wider clinical staff.
Manage patients with complex medication needs, including deprescribing where appropriate.
Support and deliver long-term condition clinics where appropriate (e.g., hypertension, asthma, diabetes) acting within scope of practice.
Deal with hospital letters where there have been medicine changes instigated by the hospital or another provider.
Identify any potential medication issues (e.g., drug interactions, inappropriate prescribing, non-adherence) and make recommendations for improvements.
Discuss the patient's understanding of their medication and offer education on proper use, potential side effects, and lifestyle considerations.
Respond to acute medicine queries from patients and healthcare professionals.
Provide patient-facing consultations via telephone, face to face or via NHS digital platforms.
Provide tailored advice regarding over-the-counter products, alternative treatments, and possible therapy adjustments.
Record and document all recommendations and patient feedback in EMIS Web.
- Appointments:
Patient appointment lengths will vary but an expectation of completing a total of 20 medication review appointments per 4 hour shift.
Appointments will be prioritised based on patient need, medication complexity, and the potential for improving health outcomes through medication management. This will be managed by the management team.
Ensure that each review includes:
Medication history review.
Assessment of medication adherence.
Identification of any adverse effects or concerns.
Suggestions for improvements or adjustments and appropriate and safe medication changes working within your clinical competency.
- Administrative Duties:
Clinical Documentation: Ensure thorough and accurate documentation of all patient interactions, including medication changes, recommendations, and changes in EMIS Web prior to session end.
Follow-up Communication: Send any necessary follow-up information or recommendations to patients via text or via reception as required.
Collaboration: Communicate with the GP team with any concerns or queries via EMIS Web task. Communicate any acute / urgent concerns with the doctor on call for that working day.
Data Entry/Reporting: Maintain accurate logs of completed appointments, recommendations, and patient progress for quality assurance and reporting purposes.
Clinical Research and Updates: Stay up-to-date with the latest clinical guidelines, drug interactions, and evidence-based practices relevant to primary care.
