Job summary
The Care Navigator is often the first point of contact for patients and visitors to Mortimer Medical Practice and plays a vital role in ensuring patients receive timely access to the most appropriate healthcare professional or service.
The postholder will provide a welcoming, efficient and professional reception and administrative service whilst using care navigation skills to support patients in accessing the right care, at the right time, from the right person.
Working within Practice policies, protocols and NHS guidance, the Care Navigator will contribute to the smooth running of the Practice by delivering excellent customer service, maintaining accurate patient records and supporting both clinical and non-clinical teams.
The role requires excellent communication skills, sound judgement, empathy, confidentiality and the ability to remain calm whilst managing competing priorities within a busy healthcare environment.
Main duties of the job
To deliver a high-quality patient experience by
- Acting as the first point of contact for patients, relatives and external agencies.
- Using Care Navigation principles to direct patients safely and appropriately to the most suitable clinician, healthcare professional or external service.
- Supporting delivery of the Practice's Total Triage model through effective management of patient contacts.
- Providing comprehensive reception and administrative support to ensure the efficient operation of the Practice.
- Maintaining accurate patient records in accordance with Practice procedures, GDPR and NHS information governance requirements.
- Promoting a positive, professional and compassionate image of Mortimer Medical Practice at all times.

Job summary
We are looking for an Operations & Compliance Lead to join our Leadership Team and play an important role in ensuring our practice remains safe, compliant, well maintained and fit for the future.
This varied and responsible role works closely with the Practice Manager and wider clinical and operational leadership team. You will take ownership of the practices operational compliance framework across all three sites, ensuring inspections, servicing, audits, risk assessments, policies and statutory requirements are completed and appropriately recorded.
You will need to keep abreast of requirements arising from CQC, legislation, NHS England, the GMS contract, local commissioning arrangements and other national requirements affecting general practice. You will be organised and proactive, able to identify what needs to be done, coordinate contractors or specialists and ensure actions are followed through.
This is not a technical maintenance role. You will coordinate the right people to undertake specialist work, from Legionella testing and fire equipment servicing to health and safety audits, equipment calibration, premises security and building maintenance, ensuring nothing is overlooked and compliance can be evidenced.
We are looking for someone practical and solutions-focused, with excellent attention to detail and the confidence to take ownership.

Main duties of the job
The Operations & Compliance Lead is a key member of the Leadership Team at Mortimer Medical Practice, responsible for ensuring the practices premises, operational systems, health and safety arrangements and compliance requirements are effectively managed across all three sites.
Reporting to the Practice Manager and working with the wider Leadership Team, you will provide proactive oversight of the practices operational and compliance framework, ensuring checks, inspections, servicing, audits, risk assessments, policies and remedial actions are completed within appropriate timescales.
You will maintain a clear understanding of requirements arising from CQC standards, relevant legislation and statutory guidance, NHS England, the GMS contract, local commissioning arrangements and other national requirements affecting general practice. You will ensure compliance can be evidenced and that identified risks and improvements are addressed promptly.
This is a practical, proactive role. You will not undertake specialist maintenance or technical work yourself, but will identify what is required, arrange appropriately qualified contractors or specialists and ensure records and certificates are maintained.
Working across Kingsland, Orleton and Leintwardine, you will help ensure Mortimer Medical Practice remains safe, secure, compliant, well maintained and fit for purpose, supporting smooth day-to-day operations and providing assurance to the Partners, Practice Manager and Leadership Team.

Operations, Premises and Facilities
- Maintain oversight of the condition, safety, security and functionality of all three practice premises.
- Coordinate and monitor practice maintenance, ensuring issues are identified, prioritised and resolved appropriately.

Main duties of the job
The Practice Manager will have overall day-to-day oversight of the practice, ensuring the effective, safe and efficient operation of all three sites. While the postholder will have support from a strong management and leadership team, they will be expected to be visible and actively involved in the day-to-day running of the practice. - Maintain oversight of CQC compliance and ensure appropriate action is taken in response to regulatory and governance requirements.

The Practice Manager will work collaboratively with the wider management team, with responsibility for specific areas delegated appropriately to other managers and leads. The postholder will retain overall operational oversight, ensuring that delegated responsibilities are effectively managed, risks are identified and escalated appropriately, and the practice operates as a coordinated and cohesive organisation.
Place of Work:
We aim for the main location of work to be at Kingsland Surgery, but there will be requirements to travel to both Orleton Surgery and Leominster Surgery as required by the needs of the business.
