Job summary
The post holder will work as a member of the Primary Care Nursing Team under the direction of the lead nurse.
The Nursing Support Worker will support Registered Nurses, General Practitioners and other health care professionals in the assessment, treatment and management of patients presenting in primary care settings.
The Nursing Support Worker will work as part of the nursing team within standard operating procedures, and Registered Nurses and General Practitioners are available for reference.
The post holder will assist with specified delegated clinical tasks that include the provision and monitoring of Chronic Disease management providing a caring, patient focused service at all times.
Main duties of the job
- Carry out delegated clinical duties within HMP & YOI Parc.
- Undertake Phlebotomy clinics and dry blood spot testing.
- To perform physiological measurements such as blood pressure monitoring, pulse monitoring, waist circumference measurement, weight monitoring.
- Providing any necessary Health Promotion advice to patients and carers e.g. Smoking Cessation, Weight Management Support, Alcohol Consumption.
- Undertake Spirometry testing, and record result in patient information.
- Undertake ECG monitoring.
- To participate in the administration of immunisations under a Patient Specific Directive following a period of training i.e. B12, influenza and pneumococcal injections.
- To perform basic wound management under the supervision of a Registered Nurse following appropriate training and assessment.
- To prepare the clinical environment for the patient ensuring the treatment area is clean, stocked and tidy

Job summary
To deliver evidence based holistic professional mental health care to people within the context of an integrated multi-disciplinary, multi-agency integrated community mental health team.
The post holder is identified as a skilled and creative practitioner with knowledge of working with and supporting people living within their own communities and will be an integral member of the Community Mental Health Team.
Acting within their professional boundaries, the post holder will provide support and intervention for the presenting person through holistic person centered assessment, clinical consideration, diagnosis, treatment and evaluation of their care within a multi-agency team approach. They will demonstrate safe, clinical decision-making and expert care and support for people within their homes.
The post holder will work collaboratively with the wider multi-disciplinary general practice teams to meet the needs of people within their community.

Main duties of the job
- To take responsibility for a clinical caseload in accordance with the Mental Health Measures.
- Foster robust working relationships with all partners.
- Contribute to the continuing improvement and development of mental health services.
We are looking for highly motivated individual to join our well established multi- disciplinary CMHT. The main purpose of this role is to deliver evidenced based, holistic care to individuals suffering with mental health issues. The post will be based in Keir Hardie Health Park with strong links with PCMHSS, the inpatient wards at Royal Glamorgan Hospital, MTCBC, our GP partners and third sector organisations.
The post holder will need to hold a first level qualification in mental health nursing and have excellent clinical knowledge and experience of mental health and the Mental Health Measure 2010. Excellent communication, listening and organisational skills are essential along with MDT working, in a challenging and changeable environment.
The successful applicant will need to demonstrate sound clinical knowledge and awareness of current clinical practice in mental health, have excellent clinical assessment skills and have sound knowledge of safeguarding procedures. The post holder will be expected to manage a caseload of patients with mental health issues, subject to CTP and will work under the leadership of a Band 7 nurse.

Job summary
An exciting opportunity has arisen for a Consultant in General Surgery with a Benign Upper GI Sub Specialty Interest.
The post holder will be based at the Royal Glamorgan Hospital, Llantrisant, South Wales. The Department of General Surgery is part of the Planned Care Directorate and spans all 3 District General Hospital sites; Prince Charles Hospital in Merthyr Tydfil, Royal Glamorgan Hospital in Llantrisant and Princess of Wales Hospital in Bridgend. The post holder may be required to work across other sites within the Health Board. The on call is 1in 8.
Although the Job description may specify a base of work a negotiation for hospital site can be considered.

Main duties of the job
The post holder will be required to share with consultant colleagues the responsibility for providing outpatient and inpatient services for the local population and in doing so, foster excellent working relationships within and outside the General Surgical department, including medical, nursing, secretarial and support staff, General Practitioners and management teams.
The post will also work with the Directorate Management team to assist in the strategic planning and modernisation of General Surgical services, deliver internal and external targets for patient activity and achieve financial balance.
The post will be part of the consultant emergency on call rota on a 1:8 basis with a 10-session job plan with enhanced payment for the on-call commitment.

We are seeking an experienced and motivated Specialist Paediatric Occupational Therapist to join our Neonatal Follow-Up Service within Cwm Taf Morgannwg UHB.
As an autonomous practitioner, you will provide expert assessment and intervention for infants with complex neonatal presentations, using highly specialised clinical reasoning skills to inform assessment, differential diagnosis, and treatment planning.
The role involves delivering advanced occupational therapy interventions and advice to infants, families, and carers, ensuring high-quality, family-centred care that supports the best possible developmental outcomes.
You will also play a key role in developing and delivering neonatal follow-up occupational therapy services across the Health Board, acting as a specialist resource and mentor for colleagues. This post will join the team responsible for the provision of all Dermatology services to the Local Health Board in Cwm Taf Morgannwg.

They will support the local delivery and evaluation of Nutrition Skills for LifeTM services.
The successful candidates will receive a range of training opportunities and work alongside fellow Dietetic Assistant Practitioners in Public Health Dietetics, under the guidance of the Senior Public Health Dietitians. They will receive weekly supervision sessions with a registered dietitian and will work closely with a range of partner agencies to promote positive healthy lifestyle behaviours.

Main duties of the job

- Provide a vital link between primary care professionals and the community they are working in, to support and empower service users to make the changes they identify to improve their health and wellbeing
