Job summary
We are looking for an experienced and proactive Health Care Assistant or experienced Nurse Associate to join our already experienced and established practice nursing team.
We would be looking for you to work between 32 - 35 hours each week.
The successful candiate would need to be motivated and committed to providing high quality care and work collaboratively with the general practice team to meet the needs of the patient.

About us
Located just outside the heart of Wigan town centre, The Dicconson Group Practice is a well established GP Practice serving a diverse patient population of approximately 9650 patients.
You will be joining a welcoming and experienced clinical team at the Practice.
This is headed up by 4 supportive GP Partners. We have a dedicated team of 3 salaried GP's . 2 Advanced Practitioners, 3 Practice Nurses, a Nurse Associate and our Healthcare Associate.
They in turn are supported by our non-clinical team, which includes our friendly reception team , dedicated secretaries and data quality lead as well as our management team of 3.
We pride ourselves on our collaborative supportive environment, where every team member is valued and encouraged to contribute to the ongoing development of the Practice.

Job summary
An exciting opportunity has arisen for a QOF Support Administrator to join the Business Intelligence team at Beacon Medical Group.
This is a six-month fixed term contract with a view to extend to a permanent position, working 26.5 hours per week.
The postholder will run their own areas of our Quality and Outcomes Framework (QOF) and similar schemes, keeping up to date with any changes and fully understanding any business rules that are published.

An exciting opportunity has arisen for Trainee Psychological Wellbeing Practitioners to join the NHS Talking Therapies Service delivered jointly by Coventry and Warwickshire Partnership Trust (CWPT) and Coventry and Warwickshire Mind. The successful candidates will be joining a well-established service that continues to offer high quality; evidence based psychological interventions for our identified populations. The posts are for the Coventry, North Warwickshire & Rugby & South Warwickshire.
The PWP training programme is a 12-month course. The programme combines clinical competency development with academic and work-based learning through close collaboration between Coventry & Warwickshire Partnership Trust and Coventry University. Successful applicants will need to commit to the academic and competency development requirements of the course.
IMPORTANT:
If you are shortlisted for the trainee role, you will be entered into a two-stage recruitment process:
- You will receive an invitation from Coventry University to submit a short written exercise. You will receive details about the exercise by 28thSeptember, and will be required to send your completed submission to the university by 4thOctober.

Main duties of the job

Support people by connecting them to non-medical, community-based activities, groups and services that meet their practical, social and emotional needs, including specialist advice services and arts and culture, physical activity, and nature and green based activities
Support people with advice and guidance to improve their mental wellbeing addressing issues such as financial issues, loneliness or social isolation, anxiety, housing etc.
Use coaching and motivational interviewing techniques to support people to take control of their own health and wellbeing

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

First Contact Mental Health appointments provided to patients registered at our four practices. Offering initial review, onward referral to external service providers, appropriate prescribing eg SSRIs and follow-up appointments. Mereside Medical is a flourishing group of three East Cambridgeshire practices in Ely, Haddenham and Soham, together serving c.45,000 patients. The Role
Over the last six years, our Clinical Pharmacy team have become a fundamental part of Mereside Medical Group Practices highly valued and appreciate by out GPs, Practice teams, and patients thanks to a track record of making a genuine difference to patient outcomes and GP workloads.
The team comprises four Clinical Pharmacists and three Pharmacy Technicians, working across three GP Practices - Cathedral Medical Centre in Ely, Haddenham Surgery, and Staploe Medical Centre in Soham.
Each Clinical Pharmacist is offered clinical supervision by a GP, and managerial supervision is offered by the Lead Pharmacist.
Your Responsibilities
Patient Care & Clinical Responsibilities
- Undertake clinical medication reviews and Structured Medication Reviews, including for care home residents and patients with long-term conditions.
- Deliver patient-facing consultations (face-to-face, telephone, or digital),providing medicines advice and managing referrals and acute queries, and assess minor conditions where appropriate.
- Offer safety-netting, lifestyle advice, and appropriate signposting.
- Work collaboratively within multidisciplinary teams to optimise patient care.
Medicines Optimisation & Prescribing
- Lead medicines optimisation to ensure safe, effective, and cost-efficient prescribing.
- Manage repeat prescribing and undertake medicines reconciliation to ensure safe transitions of care.
- Identify and manage prescribing risks and support reduction in avoidable medicines-related hospital admissions.
Clinical Governance, Quality & Safety
- Lead and support clinical audits, quality improvement, and service development.
- Use prescribing data to improve outcomes, manage high-risk patient cohorts, and implement local/national guidance.
- Support CQC compliance, incident reporting, and delivery of PCN DES, QOF, and public health priorities.
Leadership, Education & Services Development

Job summary
Ealing NHS Talking Therapies was one of the first Talking Therapies services to launch in 2008, and is one of the three services provided by West London NHS Trust. We are a large team of over 70 members of staff and are currently working on further expansion. Our main base is in Ealing where we have access to over 16 clinic rooms and two open plan offices. In addition to this, we run clinics at GP surgeries across the borough, covering areas such as Southall, Acton, and Chiswick. Our service is well-established and many of our staff have been with us since the beginning.
Ealing Talking Therapies is passionate about offering a great quality therapy to our patients and we are very proud to be able to consistently offer all the Talking Therapies-approved therapies, such as EMDR, DIT, IPT, BCT, and CfD. To that effect, we are committed to Continuing Professional Development (CPD) of our staff and training in additional Talking Therapies modalities is highly encouraged. We have close links with our neighboring services (H&F and Hounslow) and share our CPD events with them, providing an opportunity to link up with our colleagues across the Trust.
Over the last year, our staff have attended in-house training in complex PTSD, LTC, OCD, perinatal, and many others. In addition to the standard talking therapies, our service is one of only two Talking Therapies services in London that offer a weight management group for our patients
Main duties of the job
Working within team of three to cover the duty desk for the service. This involves providing advice on risk management, safeguarding and appropriate onward referrals in line with NICE and Trust guidance.
Liaising with other services and following up on referrals
Screening referrals as they come in and engaging in clinical discussions and decision making on appropriateness of referrals.
Running and developing specialist safeguarding training with the service.
Reviewing incident reports and providing feedback and learnings to clinicians on these.
Working with clients to resolve concerns or questions they may have about the service to improve treatment outcomes.
