In addition to these services there is also a PICU Mental Health Service.

The service provides a seamless internal care pathway supporting and empowering people in their recovery and rehabilitation. Working alongside the multidisciplinary team, you will provide personalised assessment and treatment programmes for males with mental illness/complex care needs as well as those with personality disorder, and females and males in a PICU service.

A social prescriber is a link worker who empowers people to take control of their health and wellbeing through giving time, focusing on what matters to me and taking a holistic approach. The post holder works to connect people to community groups as well as statutory services for practical and emotional support. Social Prescribers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.

The post holder will be deployed in a specific Primary Care Network and Enhanced Access Service, working in partnership with the practice teams in that network to support patients and promote lifestyle changes, preventing ill-health by means of behaviour change techniques. The post requires: good understanding of social prescribing, well-being, and community services and a good understanding of; and ability to provide; motivational and behavioural techniques.

Social prescribing can help to strengthen community resilience and personal resilience, and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity; and by increasing peoples active involvement with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

We recognise that existing social prescribing schemes may employ link workers on a different salary scale, however this proposed salary reflects the complexity of the situations that people present with, and the need for a significant level of multi- agency working, including supporting community groups to receive referrals.

Main purpose of the role.To support the five GP partners, to manage a busy and interesting clinical caseload. The appointee will also have the opportunity to take part in leading on key disease areas, monitoring trends in treatment, staff training and prescribing, assisting assist in developing appropriate healthcare services for a diverse, patient list, practice based commissioning, patient participation and health promotion. This is a GMS Practice.

Clinical responsibilities.

In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations and emergencies, telephone consultations and queries, triaging, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, patient test results, patient medical reports and examinations e.g. Insurance Companies on behalf of the Practice, referral letters NHS/private, paperwork and correspondence in a timely manner;

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation;

Recording clear and contemporaneous I.T. based consultation notes to agreed standards;

Collecting data for audit purposes.

Out-of-hours OOH as agreed by the partnership.

Compiling and issuing computer-generated acute and repeat prescriptions avoiding hand-written prescriptions whenever possible;

Prescribing in accordance with locally agreed or national guidelines.

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
