Job responsibilities

• Deliver chronic disease management clinics (e.g. diabetes, asthma, COPD and hypertension).
• Undertake cervical screening in line with the NHS Cervical Screening Programme.
• Administer childhood and adult immunisations and vaccinations under PGDs/PSDs.
• Provide wound care, dressings, venepuncture, NHS Health Checks, and other agreed clinical procedures.
• Promote health and wellbeing through patient education and preventative care.
• Maintain accurate patient records using EMIS Web and ensure high standards of clinical documentation.
• Support the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.
• Participate in clinical audit, quality improvement and infection prevention and control activities.
• Work collaboratively with GPs and the wider multidisciplinary team, referring patients appropriately and contributing to the delivery of high-quality patient care.

• - Experience working as a Registered Nurse within a primary care or community healthcare setting.

• - Experience managing patients with long-term conditions such as diabetes, asthma, COPD and cardiovascular disease.
• - Experience undertaking clinical assessments and recognising when escalation or referral is required.
• - Experience maintaining accurate clinical records and using electronic clinical systems (e.g. EMIS Web).
• - Experience working collaboratively within a multidisciplinary healthcare team.
• - Experience supporting quality improvement, clinical audits and meeting NHS contractual requirements.
• - Experience promoting health education, prevention and self-management with patients.

Qualifications
