Job responsibilities

The postholder will:

• Undertake face-to-face and telephone consultations, clinical triage and home visits where clinically required.
• Work within our total triage model using SystmConnect, ensuring patients are directed to the most appropriate care.
• Work to 15-minute appointments, with dedicated review clinics incorporated into the working structure.
• Assess, diagnose, investigate and manage patients presenting with acute and chronic health conditions.
• Prescribe safely and appropriately in accordance with current guidance.
• Review pathology results, clinical correspondence, medication requests and other associated clinical administration.
• Maintain accurate, timely and comprehensive patient records using SystmOne.
• Make appropriate referrals to secondary care and other community services.
• Participate in clinical meetings, significant event reviews, audits and quality improvement activities.
• Work collaboratively with GPs, Nurse Practitioners, nursing staff, ARRS professionals and the wider practice team.
• Contribute to safeguarding, clinical governance and the delivery of safe, effective services.
• Support the practice's involvement in undergraduate medical education and the development of our wider clinical team.
• Maintain professional registration, appraisal, revalidation and continuing professional development requirements.

The postholder will be expected to work in accordance with practice policies and procedures and contribute positively to the continued development of Acklam Medical Centre and the services we provide to our patients.

Person Specification

Experience

Essential

• Experience of working in General Practice
• Experience of managing a broad range of acute and chronic conditions
• Experience of clinical triage and managing patient demand safely
• Experience of working effectively within a multidisciplinary team
• Competent in using electronic clinical record systems
• Evidence of maintaining high standards of clinical care and accurate record keeping

Desirable

• Experience of working within a total triage model
• Experience of using SystmOne and/or SystmConnect
• Experience of teaching or supporting medical students or trainees
• Experience of clinical audit, quality improvement or service development
• Experience or a special interest in an area that complements the existing practice team

Qualifications

Essential

• Medical degree (MBBS, MBChB or equivalent)
• Full GMC registration with a licence to practise
• Inclusion on the NHS England Medical Performers List
• Completion of GP training with CCT in General Practice or equivalent recognised qualification
• Evidence of ongoing Continuing Professional Development (CPD)

Desirable

• Additional postgraduate qualifications or areas of special clinical interest
• Experience or qualifications in medical education, supervision or teaching
• Evidence of involvement in quality improvement, audit or service development
• Additional skills that complement the existing practice team
