Job summary
The postholder will provide high quality primary care services as part of the Enhanced Access Service, delivering care to patients across the local Primary Care Network (PCN). The role may be undertaken either through face to face consultations at designated Enhanced Access sites or via remote consultations (telephone and/or video), depending on the session booked. The postholder will ensure patients receive timely, safe, and effective care during enhanced access hours.
Main duties of the job
The GP will undertake the full range of clinical duties expected within a general practice setting, including patient assessment, diagnosis, treatment, prescribing, referrals, and the investigation and interpretation of test results. Where ongoing management or follow-up is required, this will be communicated appropriately to the patient's registered practice or relevant healthcare provider.
About us
Sutton Primary Care Networks is the collaborative vehicle for the four Sutton PCNs and the former Sutton GP Federation working together as a single entity. We are a Community Interest Company that aims to reinvest surpluses back into general practice and primary care for the benefit of our practices and the people who form the communities we serve.
Sutton has four PCNs whose boundaries are based on historical relationships and groupings of organisations. There are some similarities of residents health and care needs unique to a particular PCN, and many more needs that are shared across all four PCNs.
Our four PCN represent all 22 GP surgeries in Sutton. Their collective footprints cover all of the London Borough of Sutton and its residents who are able to access and benefit from the services and improvements offered by Primary Care Networks.
Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan, with all general practices being required to be in a network by June 2019, and CCGs/Integrated Care Boards (ICBs) being required to commit recurrent funding to develop and maintain them.

Clinical Duties
- Provide face to face, telephone, and remote consultations within the Enhanced Access Service.
- Assess patients presenting with urgent and routine healthcare needs.
- Undertake clinical assessment, diagnosis, and treatment of patients presenting with acute and long-term health conditions.
- Analyse and interpret patient history, presenting symptoms, examination findings, and diagnostic information to develop appropriate differential diagnoses.
- Prioritise clinical problems and initiate appropriate treatment, including emergency intervention where required.
- Formulate evidence-based management plans in line with national and local clinical guidelines.
- Prescribe medication in accordance with local and national prescribing guidelines.
- Ensure prescribing decisions are clinically appropriate, safe, evidence-based, and cost-effective.
- Make referrals to secondary care, community services, and other healthcare professionals where appropriate.
- Liaise with community pharmacies where necessary regarding patient safety and prescribing requirements.
- Provide health promotion, preventative care, and self-management advice.
- Issue fit notes, medical certificates, and other clinical documentation within professional competence and local policies.
- Assess and provide appropriate treatment, advice, and onward referral for patients presenting with complex physical and mental health needs, ensuring safe handover to the patient's registered practice where ongoing management is required.
- Escalate significant clinical concerns, incidents, and risks appropriately.
- Participate in safeguarding processes and act upon safeguarding concerns in line with local and national guidance.
- Ensure patients are managed in a safe, compassionate, and patient centred manner at all times.
Remote Consultation Responsibilities
- Conduct safe and effective remote consultations via telephone and approved digital consultation platforms.
- Demonstrate effective communication skills when consulting remotely, ensuring patients understand diagnosis, treatment plans, medication advice, safety-netting, and next steps.
- Assess and manage clinical risk during remote consultations, recognising when a face-to-face assessment is clinically required.
- Arrange escalation or onward referral where remote consultation is not appropriate or sufficient for safe patient care.
- Utilise EMIS Web, Accurx, and other approved digital systems effectively to support patient care.
Clinical Governance and Administration
- Maintain accurate, complete, and contemporaneous clinical records on the appropriate clinical system.
- Ensure compliance with Information Governance, GDPR, confidentiality, and Caldicott principles.
- Undertake mandatory training and maintain continuing professional development in line with GMC requirements.
- Comply with all relevant PCN, Enhanced Access, NHS, safeguarding, confidentiality, infection prevention and control, and health and safety policies.
- Provide clinical advice and support to colleagues where appropriate.
