We operate a Total Triage model via GP Triage, where all appointment requests are assessed at the first point of contact and signposted appropriately. Our multidisciplinary team includes a broad mix of GPs,and nurses,bringing a wide range of expertise and perspectives to the care we provide.

Our nursing team plays a central role in the management of patients with chronic conditions, ensuring continuity of care and supporting improved health outcomes. In addition, we provide a GP-led L2 Sexual Health Service to our patients.

Job Purpose

The post holder will provide comprehensive general medical services to patients registered with the practice.

The post holder will be responsible for the assessment, diagnosis, treatment and ongoing management of patients presenting with acute and chronic medical conditions, working within their professional competence and in accordance with GMC standards, NHS requirements and practice policies.

Key Responsibilities

Clinical Responsibilities

The post holder will:

  • Manage GP Triage dashboard / online consultations
  • Provide consultations to patients presenting with acute, chronic and undifferentiated conditions.
  • Undertake clinical assessment, diagnosis and management of patients.
  • Provide face-to-face, telephone and other appropriate forms of consultation.
  • Prescribe safely and appropriately in accordance with current clinical guidance.
  • Refer patients appropriately to secondary care, community services and other healthcare professionals.
  • Arrange appropriate investigations and interpret results within the scope of professional competence.
  • Provide appropriate follow-up and safety-netting advice.
  • Undertake management and review of patients with long-term conditions as required.
