Job summary
To work as part of the multidisciplinary primary care team, providing high-quality, evidence-based nursing care to the practice population. The Practice Nurse will undertake chronic disease management, health promotion, screening, immunisation programmes, cervical cytology, wound care, and other clinical duties to improve patient health outcomes whilst ensuring safe and effective service delivery.
Main duties of the job
The main duties of the role will be to deliver annual reviews to the long term condition patients, vaccinations, cervical smears, phlebotomy, stock ordering on immform. The Practice Nurse will also be involved in our Practice Delivery Agreement with our PCN to ensure we meet the specified targets and will also be involved in meeting QOF targets. The practice has a weekly clinical meeting where the Practice Nurses are expected to attend.
About us
We are a fairly large, friendly practice based near the centre of Barnsley. We have a current list size of approximately 11300 patients and are currently taking on new patients to the list. We have a reliable and friendly team of staff including GPs, Advanced Practitioners, Physician Associates, Practice Nurses, Nurse Associate, Health and Wellbeing Coaches, Care Coordinators, Nurse Coordinator, Secretaries, Receptionists and Maintenance workers.
