Job summary

We are a friendly and supportive training practice located on the outskirts of Halifax, West Yorkshire and we have a very exciting opportunity for a motivated, committed and experienced Care Navigator to join the team for 25 hours per week.

Main duties of the job

Maintaining and monitoring the practice appointment system

Process personal, telephone and e-requests for appointments

Answer incoming phone calls, transferring calls or dealing with the callers request appropriately

Signpost patients to the correct service

Initiating contact with and responding to, requests from patients, team members and external agencies

Photocopy documentation as required

Data entry of new and temporary registrations and relevant patient information as required

Input data into the patients healthcare records as necessary

Direct requests for information i.e. SAR, insurance / solicitors letters and DVLA forms to the administrative team

Manage all queries as necessary in an efficient manner

Maintain a clean, tidy, effective working area at all times

Monitor and maintain the reception area and notice boards

Support all clinical staff with general tasks as requested

About us

The team includes six GP Partners, three Salaried Gps, four Advanced Clinical Practitioners who are supported by a full Nursing Team comprising of four Practice Nurses, one Nursing Associate, one Healthcare Assistant, two Phlebotomists as well as Pharmacy Prescribers. The practice also has First Contact Physiotherapy and Podiatry services running from the practice.
